Absence Of Villi In Duodenal

Loey

Yesterday I finally had the biopsy of my ulcer (EGD) and the doctor came to the following conclusions:

Esophageal Hiatal Hernia

Scalloped Folds and absence of villi in the duodenal

Ulcer in the pre-pyloric region

I'll be taking Omeprazole once a day for a month and then have to have the procedure repeated.

I'm concerned about the absence of villi in the duodenal. I'm familiar with them being damaged with Celiac and then healing over time but had never heard a case of them being absent. Does anyone have experience with this?

Thanks!

Loey

Results of biopsy will take about a week.

They aren't totally gone - they're just flat, which means that your cells have to rebuild the structures from scratch. They will, but it'll take a significant length of time. The cells are made at the bottom of the heap, then are pushed upward and into the villous structure by new cells formed beneath them (very similar to the way skin is formed, actually, but into a different end structure). This means that you're down to only baby cells. As your body stops attacking the cells, you'll ultimately get back to having a normal gut structure. The biggest down side to the way your gut is right now is that the surface area of your gut is greatly decreased. The villi are there to increase surface area so you can absorb all the nutrients from your food, so without that increased surface area, you'll still be at risk for nutrient deficiencies, especially of the fat-soluble vitamins (A, D, E, and K).

Keep on a multivitamin, take calcium + D supplements, eat a healthy diet, be strict about being gluten-free, and you'll get back to a normal structure. It's just going to take some time.

Absence of discernible villi is Marsh IV, the most advanced stage of celiac damage. In most cases the villi will regenerate, but as said above, it will take time.

Your situation sounds similar to mine in June of 2000. A repeat scope in 2005 showed normal villi. My healing process took several months, although symptoms improved significantly in just a couple of weeks.

no antibiotics for the ulcer? most ulcers are caused by H.Pylori bacteria...

hope they have ruled that out or are going to actually treat it, so it doesnt come back months from now.

happy you have answers now!!! time to get healing :)

You finally got the biopsy! Good going for sticking with it Loey. sounds to me like the amarperazole is a stomach acid reducer? Well, maybe they wil put you on antibiotics after the biopsy results are back. I have read they give people a triple package of anti-biotics, 3 different kinds to kill the h.pylori.

Here is some info from wiki on diet related treatments. They are not a cure for h.pylori, but some help.

Studies have recently been published suggesting H. pylori activity could be suppressed via dietary methods. A 2009 Japanese study in Cancer Prevention Research found that eating as little as 70 g (2.5 ounces) of broccoli sprouts daily for two months reduces the number of colonies of H. pylori bacteria in the stomach by 40% in humans. This treatment also seems to help by enhancing the protection of the gastric mucosa against H. pylori, but is relatively ineffective on related gastric cancers. The previous infection returned within two months after broccoli sprouts were removed from the diet, so an ongoing inclusion in the diet is best for continued protection from H. pylori.[39]

A 2008 study published in Korean Journal of Microbiology and Biotechnology found kimchi (fermented cabbage) contains a bacterial strain "showing strong antagonistic activity against H. pylori." The bacterium strain isolated from kimchi, designated Lb. plantarum NO1, was found to reduce the urease activity of H. pylori by 40-60% and suppress its binding to human gastric cancer cell line by more than 33%.[40]

Good point. My doctor said my digestive track can't handle the three antibiotics needed and that the success rate is only 70%. Instead, she had me on six different supplements that include oregano, garlic, L-glutamine, aloe, and a bunch more that I can't recall. For the first few days of taking them, I notice no difference. I'm happy to report that I no longer have the pain, burning and gnawing in my stomach.

P.S. I also have a hiatal hernia.

They just took the blood work at the hospital for the H-Pylori. I imagine if it shows up he will add the antibiotics. The jury is still out on how I feel about this doctor. I have a referral from a colleague of my husband's if Im not happy.

Thanks for your help and support

i had an ulcer around April this year- i did GSE 3 doses a day for 6 weeks and Zantac & Pepto as needed... and it worked. (GSE is antibacterial, antifungal, antiinflammatory, etc). oh, i also finished off my Ionized silver in that first 2 week period.

i always go full on with GSE & Ionized Silver in the beginning to see if it works- because i DESPISE antibiotics. 99% of the time- if i go on Antibiotics- i will FOR SURE end up with a Sore Throat and a Yeast Infection- without a doubt-

also i read that broccoli sprouts will discourage H.Pylori from setting up camp. i ate those off and on this summer. but now with hypothyroid, i guess i have to avoid them :(

Is that something you can get over the counter or at a health food store? Also, are broccoli sprouts found where regular sprouts are (sorry to sound ignorant).

Getting ready to leave overnight for a funeral. Have to pack up my food. Would NOT want to forget that! I'll be back tomorrow night or Tuesday.

Thank you all for always having a great suggestion, idea or link. You are all the best!!!!

Welcome to the forum, @SuzanneL! Which tTG was that? tTG-IGA? tTG-IGG? Were there other celiac antibody tests run from that blood draw? Was total IGA measured? By some chance were you already cutting back on gluten by the time the blood draw was taken or just not eating much? For the celiac antibody tests to be accurate a person needs to be eating about 10g of gluten daily which is about 4-6 pieces of bread.

I've recently received a weak positive tTG, 6. For about six years, I've been sick almost everyday. I was told it was just my IBS. I have constant nausea. Sometimes after I eat, I have sharp, upper pain in my abdomen. I sometimes feel or vomit (bile) after eating. The doctor wanted me to try a stronger anti acid before doing an endoscopy. I'm just curious if these symptoms are pointing towards Celiac Disease?

I'm sorry to hear about what you're going through. It must be really frustrating and uncomfortable. Have you talked to your healthcare provider about these sudden sweating episodes? It might be helpful to discuss this with them to rule out any other underlying issues or to see if there are specific strategies or treatments that can help manage this symptom. Additionally, staying hydrated and wearing breathable clothing may provide some relief.
