    Alcohol Can Trigger Gluten Sensitivity in Genetically Susceptible Individuals

    Photo: CC-- Magic RobotA team of researchers recently set out to investigate the prevalence of gluten-related serological markers in patients with alcohol-related ataxia, and to compare the pattern of brain involvement on magnetic resonance imaging between patients with alcohol and gluten ataxias.

    The research team included Stuart Currie, Nigel Hoggard, Matthew J. R. Clark, David S. Sanders, Iain D. Wilkinson, Paul D. Griffiths, and Marios Hadjivassiliou.

    They are variously affiliated with the Academic Unit of Radiology at the University of Sheffield, and with the Department of Gastroenterology, and the Department of Neurology of Sheffield Teaching Hospitals NHS Foundation Trust in Sheffield, United Kingdom.

    For their study, the team used a retrospective review of patients attending an outside clinic to identify patients diagnosed with alcohol and gluten ataxias.

    The team recorded HLA genotype and serological markers of gluten-related disorders. They also conducted cerebellar volumetry, MR spectroscopy and voxel-based morphometric analyses on the patients and compared the results to matched control data.

    Results from 904 registered patients showed 104 patients with alcohol ataxia, and 159 patients with gluten ataxia. The data also showed that 61% of the patients with alcohol ataxia, and 70% of the patients with gluten ataxia had HLA DQ2/DQ8 genotype compared to just 30% of people with healthy local blood donors.

    Interestingly, 44% of patients with alcohol ataxia showed antigliadin antibodies compared to 12% in the healthy local population and 10% in patients with genetically confirmed ataxias. None of the patients with alcohol ataxia and antigliadin antibodies had celiac disease, while 40% in patients with gluten ataxia had celiac disease.

    Patients with alcohol ataxia who had antigliadin antibodies showed patterns of structural brain abnormality that were different from gluten ataxia, but were the same as those common in alcohol ataxia.

    The results show that alcohol related cerebellar degeneration can trigger gluten sensitivity in certain genetically susceptible individuals. Such sensitivity might be caused directly by the cerebellar impact of alcohol, but factors other than duration and amount of exposure to alcohol may also be responsible for the resulting sensitivity to gluten.

    Further study is needed to nail down the exact mechanisms at work here, but this is certainly an interesting study.

    One of my first symptoms before celiac was diagnosed, or I was even feeling that sick was alchol related. Having one glass of wine in the evening was causing me to feel chills and feverish, and like I might be sick, about 5 hours later. As it was only one glass, it was a while before I connected alcohol and being sick. Three years gluten-free, and knowing 3 people with vineyards who often offer wine, I can now have a small glass of wine again. But only one small glass, about 4 ounces, or I do not feel good.

    BACKGROUND: There are various, well-documented, duodenal endoscopic markers caused by the villous atrophy of celiac disease. Another abnormality seen in association with villous atrophy, erosions in the second part of the duodenum, is described. To our knowledge, this finding has not been heretofore described in patients with celiac disease.
    METHODS: Five patients with celiac disease and erosions were encountered over a period of 2 years.
    OBSERVATIONS: The erosions were multiple, superficial, and present in the second part of the duodenum but not the duodenal bulb. All 5 patients had findings typical of celiac disease (iron deficiency, osteopenia/osteoporosis), and 4 had at least one other endoscopic marker: scalloped duodenal folds (3), fold loss (2), or mosaic pattern mucosa (2). These patients represented 7% of new cases of celiac disease during the same time period. This pattern of erosion was not observed in over 1200 other patients undergoing upper endoscopy during the study period.
    CONCLUSIONS: In a European population, the finding of erosions confined to the second part of the duodenum is specific for villous atrophy, although sensitivity is low. Erosions in the second part of the duodenum should be added to the list of endoscopic markers of celiac disease.

    Celiac.com 12/08/2005 – Researchers in the United Kingdom conducted a systematic review and meta-analysis of 15 studies published between 1966 and 2004 that evaluated the association between breast-feeding and celiac disease. Their review covered more than 4,000 children and found that breast-feeding may offer protection against the development of celiac disease, especially if it is prolonged and covers the period when gluten is introduced. It was unclear, however, whether breast-feeding merely delays the onset of symptoms, or actually offers permanent protection against the disease, and more long-term prospective cohort studies will be necessary to make such a determination.

    Celiac.com 05/18/2009 - People with clinical irritable bowel syndrome (IBS) suffer from biopsy-proven celiac disease at rates that are more than four times higher than in non-IBS control subjects, according to the results of a recent systematic review and meta-analysis conducted by Alexander C. Ford, MBChB, MD, MRCP, from Health Sciences Centre, McMaster University, Hamilton, Ontario, Canada, and colleagues.
    Prior studies have indicated that people with IBS had higher rates of celiac disease, but evidence has not been clear, and medical guidelines do not always call for celiac screening in these individuals.
    To determine rates of celiac disease in random adults meeting clinical criteria for IBS, the research team reviewed MEDLINE from 1950 to May 31, 2008, and EMBASE from 1980 to May 31, 2008. They isolated case series and case-control studies that contained data for celiac disease blood screens. They found 14 such studies.
    From each study, they isolated and aggregated positive serologic test results for celiac disease and biopsy-proved celiac disease. They then compared the data to that for patients with IBS and control individuals, using an odds ratio (OR) and 95% confidence interval (CI).
    The team isolated 4204 suitable cases from the identified studies. Of those, 2278 met clinical criteria for IBS (54%). The overall rate of positive immunoglobulin A (IgA)–class antigliadin antibodies (AGA) was 4.0% (95% CI, 1.7% – 7.2%), the rate of positive endomysial antibodies (EMA) was 1.63% (95% CI, 0.7% – 3.0%), and the rate of tissue transglutaminase (tTGA) was 4.1% (95% CI, 1.9% – 7.0%). For biopsy-proven celiac disease, the overall rate was 4.1% (95% CI, 1.9% – 7.0%).
    In patients who met the clinical criteria for IBS compared with non-IBS control subjects, aggregate OR for positive IgA-class antigliadin antibodies was 3.40 (95% CI, 1.62 – 7.13), aggregate OR for either positive EMA or tTGA was 2.94 (95% CI, 1.36 – 6.35), and aggregate OR for biopsy-proved celiac disease was 4.34 (95% CI, 1.78 – 10.6).
    The study did have some weaknesses, including issues with the methodology governing study selection, possible spectrum bias in case-control studies, possible selection bias in studies based in secondary care, and, in some cases, results too limited to allow meaningful aggregation of data.
    Still the research team concludes that rates of biopsy-proven celiac disease are more than four times higher for IBS patients than for non-IBS controls. The team recommends that, if screening is undertaken, EMA or tTGA testing be used in lieu of IgA-AGA testing due to their higher positive predictive value, though they admitted that results will depend on celiac rates in the population being screened.

    Celiac.com 06/23/2009 - Fibroblasts are one of the most important components in inflammation and tissue remodeling process, and are thought to be involved in the pathogenesis of autoimmune disorders such as celiac disease.
    Attempts to better understand the role played by fibroblasts in celiac diseases have been hampered by the absence of specific models.
    A team of researchers recently set out to isolate and culture primary fibroblasts from endoscopic duodenal biopsies of celiac and non-celiac subjects, and to analyze their growth patterns and any morphometric characteristics.
    The research team obtained 60 tissue samples via duodenal biopsy from 20 celiac patients and 114 samples from 38 non-celiac subjects. The team then mechanically chopped and enzymatically digested in order to obtain primary cell cultures.
    Using cultured cells, the researchers charted growth patterns, karyotype (Q-banding analysis), expression of typing proteins (fibroblast surface protein and cytokeratin 20) and morphometric parameters (diameters and their ratio, perimeter, area and perimeter/area ratio at computerised image analysis)
    They were able to successfully culture primary cells in 78% of the collected duodenal biopsies. Cultured cells, expressing the fibroblast surface protein, tested negative for cytokeratine 20 and maintained a normal kariotype. Cells grew slowly with no differences between the celiac and the non-celiac group. Morphometric analysis of celiac fibroblasts showed substantially increased size, with a preserved diameters ratio, and a reduced perimeter/area ratio.
    For the first time, researchers have shown the feasibility of culturing primary fibroblast cells from endoscopic duodenal biopsies in celiac and non-celiac subjects.
    This ability shows promise for enhancing studies intended to establish the role of fibroblasts as a possible partaker in the pathogenesis of the celiac mucosal damage.

    Celiac.com 06/23/2018 - If you’re looking for a great gluten-free Mexican-style favorite that is sure to be a big hit at dinner or at your next potluck, try these green chili enchiladas with roasted cauliflower. The recipe calls for chicken, but they are just as delicious when made vegetarian using just the roasted cauliflower. Either way, these enchiladas will disappear fast. Roasted cauliflower gives these green chili chicken enchiladas a deep, smokey flavor that diners are sure to love.
    Ingredients:
    2 cans gluten-free green chili enchilada sauce (I use Hatch brand) 1 small head cauliflower, roasted and chopped 6 ounces chicken meat, browned ½ cup cotija cheese, crumbled ½ cup queso fresco, diced 1 medium onion, diced ⅓ cup green onions, minced ¼ cup radishes, sliced 1 tablespoon cooking oil 1 cup chopped cabbage, for serving ½ cup sliced cherry or grape tomatoes, for serving ¼ cup cilantro, chopped 1 dozen fresh corn tortillas  ⅔ cup oil, for softening tortillas 1 large avocado, cut into small chunks Note: For a tasty vegetarian version, just omit the chicken, double the roasted cauliflower, and prepare according to directions.
    Directions:
    Heat 1 tablespoon oil in a cast iron or ovenproof pan until hot.
    Add chicken and brown lightly on both sides. 
    Remove chicken to paper towels to cool.
     
    Cut cauliflower into small pieces and place in the oiled pan.
    Roast in oven at 350F until browned on both sides.
    Remove from the oven when tender. 
    Allow roasted cauliflower to cool.
    Chop cauliflower, or break into small pieces and set aside.
    Chop cooled chicken and set aside.
    Heat 1 inch of cooking oil in a small frying pan.
    When oil is hot, use a spatula to submerge a tortilla in the oil and leave only long enough to soften, about 10 seconds or so. 
    Remove soft tortilla to a paper towel and repeat with remaining tortillas.
    Pour enough enchilada sauce to coat the bottom of a large casserole pan.
    Dunk a tortilla into the sauce and cover both sides. Add more sauce as needed.
    Fill each tortilla with bits of chicken, cauliflower, onion, and queso fresco, and roll into shape.
    When pan is full of rolled enchiladas, top with remaining sauce.
    Cook at 350F until sauce bubbles.
    Remove and top with fresh cotija cheese and scallions.
    Serve with rice, beans, and cabbage, and garnish with avocado, cilantro, and sliced grape tomatoes.

When you’re on holiday in a new and unknown environment, this can be near impossible. As awareness of celiac disease grows around the world, however, more and more cities are opening their doors to gluten-free lifestyles, none more so than the 10 locations on the list below.
    Perhaps unsurprisingly, the U.S is a hotbed of gluten-free options, with four cities making the top 10, as well as the Hawaiian island of Maui. Chicago, in particular, is a real haven of gluten-free fare, with 240 coeliac-safe eateries throughout this huge city. The super hip city of Portland also ranks highly on this list, with the capital of counterculture rich in gluten-free cuisine, with San Francisco and Denver also included.     Celiac.com 06/21/2018 - Would you buy a house advertised as ‘gluten-free’? Yes, there really is such a house for sale. 
    It seems a Phoenix realtor Mike D’Elena is hoping that his trendy claim will catch the eye of a buyer hungry to avoid gluten, or, at least one with a sense of humor. D’Elena said he crafted the ads as a way to “be funny and to draw attention.” The idea, D’Elena said, is to “make it memorable.” 
    Though D’Elena’s marketing seeks to capitalizes on the gluten-free trend, he knows Celiac disease is a serious health issue for some people. “[W]e’re not here to offend anybody….this is just something we're just trying to do to draw attention and do what's best for our clients," he said. 
    Still, the signs seem to be working. D'elena had fielded six offers within a few days of listing the west Phoenix home.
    "Buying can sometimes be the most stressful thing you do in your entire life so why not have some fun with it," he said.

    Celiac.com 06/20/2018 - Currently, the only way to manage celiac disease is to eliminate gluten from the diet. That could be set to change as clinical trials begin in Australia for a new vaccine that aims to switch off the immune response to gluten. 
    The trials are set to begin at Australia’s University of the Sunshine Coast Clinical Trials Centre. The vaccine is designed to allow people with celiac disease to consume gluten with no adverse effects. A successful vaccine could be the beginning of the end for the gluten-free diet as the only currently viable treatment for celiac disease. That could be a massive breakthrough for people with celiac disease.
    USC’s Clinical Trials Centre Director Lucas Litewka said trial participants would receive an injection of the vaccine twice a week for seven weeks. The trials will be conducted alongside gastroenterologist Dr. James Daveson, who called the vaccine “a very exciting potential new therapy that has been undergoing clinical trials for several years now.”
    Dr. Daveson said the investigational vaccine might potentially restore gluten tolerance to people with celiac disease.The trial is open to adults between the ages of 18 and 70 who have clinically diagnosed celiac disease, and have followed a strict gluten-free diet for at least 12 months.
