Celiac.com 10/12/2023 - Celiac disease is a chronic, inflammatory disease triggered by the consumption of gluten-containing foods. Early diagnosis and proper management with a gluten-free diet can reduce its impact on patients' quality of life. People with a family history of celiac disease are at a higher risk, making it crucial to actively identify cases within this group. A new study conducted by a team of researchers in Australia offers new insight into rates of celiac disease, particularly among first-degree relatives of individuals with the condition. The research team included Richard Muir, Anuj Sehgal, Jason A Tye‐Din and A James M Daveson. They are variously affiliated with The Wesley Hospital, Brisbane, QLD; St Andrew's War Memorial Hospital, Brisbane, QLD; the Wesley Research Institute, Brisbane, QLD; The Walter and Eliza Hall Institute of Medical Research, Melbourne, VIC; The Royal Melbourne Hospital, Melbourne, VIC; and The University of Queensland, Brisbane, QLD. Celiac Disease Affects Approximately 1.4% of People Globally Their research found that celiac disease affects approximately 1.4% of both the global and Australian populations. The study, conducted at the Wesley Research Institute in Brisbane, targeted first-degree relatives of individuals already diagnosed with celiac disease. These relatives were invited to participate in the research, and the study involved genetic testing for celiac disease risk alleles (HLA‐DQ2/8/7 polymerase chain reaction genotyping) and serological tests to detect specific antibodies related to celiac disease. Key findings from the study include: High Celiac Disease Susceptibility Among the participants, 86% had celiac disease susceptibility haplotypes, with 50% of children and 53% of adults carrying high-risk celiac-associated genotypes. Serology Results Sixteen individuals with susceptibility haplotypes tested positive for serological markers associated with celiac disease. Biopsy Confirmation Small bowel biopsies were performed on individuals with positive serological results, confirming celiac disease in seven children and two adults who had high-risk alleles. Celiac Disease Prevalence Among Child First-Degree Relatives Between 11% and 14% Among child first-degree relatives, the estimated rates of celiac disease was 11%, rising to 14% among those with celiac disease susceptibility haplotypes. In contrast, only 1.4% of adult first-degree relatives were confirmed to have celiac disease. These findings highlight the importance of active case finding, especially among first-degree relatives of individuals diagnosed with celiac disease. Such screening can lead to early diagnosis and timely intervention, improving the quality of life and health for celiacs. However, the study also acknowledges limitations, including the fact that not all individuals with positive serological results underwent small bowel biopsies. Additionally, the research was conducted at a single center, and a non-first-degree relatives group was not included for comparison. Overall, the study's results support international guidelines recommending active case finding among first-degree relatives of individuals with celiac disease. That's leading to some evolution, and even some overlap, within these categories. According to a SPINS 2019 “State of the Industry” report, paleo and grain-free food and beverages are showing the fastest growth in conventional retail. The paleo diet has nothing to do with food intolerance, but instead touts the perceived healthiness of certain "paleo" foods. Paleo-positioned foods grew 48.8% to $294 million in conventional retail sales compared to the previous year, while the grain-free category grew 81.5% to $139.4 million over the previous year. The biggest growth was seen in the chip, pretzel, and snack categories, which saw paleo products grow 163.5% to $41.1 million in sales and grain-free snack sales grow 258.3% to $29.7 million. Consumers who do not suffer from celiac disease, which affects only about 1 in 100 people, are driving the growth of gluten-free and paleo foods, in part because of the belief that such foods are healthier and more nutritious than their gluten-containing counterparts. Still, the exact global prevalence of celiac disease remains unknown. To get better data on that issue, a team of researchers recently conducted a comprehensive review and meta-analysis to get a reasonably accurate estimate the global prevalence of celiac disease. The research team included P Singh, A Arora, TA Strand, DA Leffler, C Catassi, PH Green, CP Kelly, V Ahuja, and GK Makharia. They are variously affiliated with the Division of Gastroenterology and Hepatology, Beth Israel Deaconess Medical Center, Boston, Massachusetts; Lady Hardinge Medical College, New Delhi, India; Innlandet Hospital Trust, Lillehammer, Norway; Centre for International Health, University of Bergen, Bergen, Norway; Division of Gastroenterology and Hepatology, Beth Israel Deaconess Medical Center, Boston, Massachusetts; Gastroenterology Research and Development, Takeda Pharmaceuticals Inc, Cambridge, MA; Department of Pediatrics, Università Politecnica delle Marche, Ancona, Italy; Department of Medicine, Columbia University Medical Center, New York, New York; USA Celiac Disease Center, Columbia University Medical Center, New York, New York; and the Department of Gastroenterology and Human Nutrition, All India Institute of Medical Sciences, New Delhi, India. For their review, the team searched Medline, PubMed, and EMBASE for the keywords ‘celiac disease,’ ‘celiac,’ ‘tissue transglutaminase antibody,’ ‘anti-endomysium antibody,’ ‘endomysial antibody,’ and ‘prevalence’ for studies published from January 1991 through March 2016. The team cross-referenced each article with the words ‘Asia,’ ‘Europe,’ ‘Africa,’ ‘South America,’ ‘North America,’ and ‘Australia.’ They defined celiac diagnosis based on European Society of Pediatric Gastroenterology, Hepatology, and Nutrition guidelines. The team used 96 articles of 3,843 articles in their final analysis. Overall global prevalence of celiac disease was 1.4% in 275,818 individuals, based on positive blood tests for anti-tissue transglutaminase and/or anti-endomysial antibodies. The pooled global prevalence of biopsy-confirmed celiac disease was 0.7% in 138,792 individuals. That means that numerous people with celiac disease potentially remain undiagnosed. Rates of celiac disease were 0.4% in South America, 0.5% in Africa and North America, 0.6% in Asia, and 0.8% in Europe and Oceania; the prevalence was 0.6% in female vs 0.4% males. Celiac disease was significantly more common in children than adults. This systematic review and meta-analysis showed celiac disease to be reported worldwide. Blood test data shows celiac disease rate of 1.4%, while biopsy data shows 0.7%. The prevalence of celiac disease varies with sex, age, and location. This review demonstrates a need for more comprehensive population-based studies of celiac disease in numerous countries. The 1.4% rate indicates that there are 91.2 million people worldwide with celiac disease, and 3.9 million are in the U.S.A. Celiac.com 08/30/2011 - In a first of its kind study, a team of researchers is attempting a global estimate of the burden of celiac disease in childhood, and to to determine what role childhood celiac disease might play in global mortality due to diarrhea. The research team included Peter Byass, Kathleen Kahn, and Anneli Ivarsson. They are affiliated with the Umeå Centre for Global Health Research, Department of Public Health and Clinical Medicine at Umeå University in Umeå, Sweden, and with the MRC/Wits Rural Public Health and Health Transitions Research Unit (Agincourt), School of Public Health, Faculty of Health Sciences at University of the Witwatersrand in Johannesburg, South Africa. In the last several decades, celiac disease has become an an increasingly recognized public health problem. More recently, celiac disease has emerged as a global earth issue, in spite scant globally representative epidemiological data. Because children with celiac disease often have chronic diarrhea and malnutrition, a proper diagnosis is often missed, especially in poorer settings, where water-borne infectious diarrheas are common, and many children fail to thrive. To make their assessment, the two used available data to build a basic model of childhood celiac disease, incorporating estimates of population prevalence, probability of non-diagnosis, and likelihood of mortality among undiagnosed children of all countries from 1970 to 2010. In their paper, the two state the assumptions underlying their model, and make the model available as a supplementary file. Based on their model, in 2010 there were around 2.2 million children under 5 years of age living with celiac disease, while each year, there would be about 42,000 deaths related to celiac disease in these children. That would mean that, in 2008, deaths related to celiac disease likely totaled about 4% of all childhood diarrhea deaths worldwide. Even if celiac disease accounts for only a small proportion of global diarrhea deaths, these deaths are preventable, but not by normal diarrhea treatment, which can often involve gluten-based food supplements.
