Question About Dh Testing...

Ok, I'm not exactly sure the right way to ask my question. Me and my girls all get an itchy rash when we're accidentally glutened but it doesn't typically seem to last all that long. Is there a way to test for DH if there isn't an active lesion? Also, if we did get tested when there's an active rash would it be accurate since we're on a strict gluten free diet?

You can get false negatives with the DH rash biopsy if you are gluten free. And sometimes it is a false negative even if you are consuming gluten.

I don't they can biopsy unless you have a rash...and the biopsy would be taken from the clear skin next to the lesion.

Hmmm wonder if IgA antibodies could be detected in the clear skin where a rash used to be?

But I doubt any physician would order or perform that biopsy.

Long ago Dr.'s would diagnose DH by placing a Iodine soaked bandage over the area for 24 hrs. and if a rash developed it was diagnostic of DH. I don't know if it works though.

I suppose the value of a test would to eliminate some 'other' skin condition or allergy.

Many of us are happy to know that if there is a 1:1 corelation between gluten and rash, then we've got DH.

Thanks for the replies. I'm pretty positive DD2's rash is DH. Last week we got some cross contamination and I got hives and a couple rashy bumps on my face and DD1 was itching her stomach and back and got a little bit of a rash on her bottom, but DD2's was the worst! She got this awful rash all over her but that she was clawing at and the other morning she woke up and was crying pointing to her diaper, I took it off and saw several blisters. I think I'm going to hold off on any testing for now since she's only 21 months but it is confirmation for sure that this gluten intolerance she has is serious. Thanks again!

Definitely worth speaking to your gastroenterologist about this. My own told me that by using Gaviscon a barrier forms over the contents of the stomach and stops gas and acid irritating the throat.  In fact, he said to me that because I found relief using Gaviscon that was a very clear indicator that reflux was the cause of that particular issue.   A wedge pillow will really help with this - or raising the top bed legs with bricks.

I did get the pneumonia vaccine about 4 years ago. I had this amazing allergist who did all those vitamin deficiencies test and told to get that vaccine. Unfortunately she retired.  I haven’t been to an allergist in a few years,  I’m not sure what my levels are now. I did have a pulmonologist who wasn’t concern and said I seemed fine to him that I was young etc. But yes I think I should at the very least get a different opinion. Thank you for your reply

Yes I do have acid reflux. I’m not on anything for it at the moment. I sometimes wonder if that’s what it could be because I get heart burn every night. I may revisit my gastrointestinal doctor again. Thanks for the reply

Hi @HelloFlowersGoodbyeFlour I wonder if you suffer from reflux, as if you do, you may find it could also be irritating your airways.  I shall explain: I have to use a blue inhaler from time to time, and it seems to be related to reflux.  Never had any trouble before my coeliac diagnosis, the reflux seemed to be something that developed following a holiday to France in 2019, where I had been exposed to gluten.    The reflux continued into the autumn and winter, my throat itched to begin with, particularly after meals, but it then that feeling of irritation seemed to spread to my lungs.  I even found it difficult to breathe on occasion. What stopped it in its tracks was using a wedge pillow at night, following a reflux diet (you can find them online), not eating 2-4 hours before bed and also having a dose of Gaviscon Advanced at night, which forms a barrier so that acid/food can't go back up your esophagus.  The throat irritation faded, and then I found it easier to breathe again. Just mentioning in case it could be a contributing factor.

Since initially getting your D checked a few years ago, has it since rebounded to normal levels? Sounds like at some point you got it checked again.
