The Proper Way To Get Tested For Dh? Are Doctors Being Ethical?

I have been gluten-free for 4 months, but I have cheated along the way. I went to a Dermatologist to get a biopsy from a rash I had on my back and arms, but by the time I was able to get the appointment, my rash was gone. The doctor was short with me, she did one biopsy from my arm and left the room. Ten days later I get another bad rash on my knee. I go back to the same practice, but saw her assistant instead. She did two biopsy's on my leg. One on the outside of the rash and another located in the middle of it. Was the doctor I saw before unethical for taking that one biopsy of my arm when the rash was no longer present? Should I be paying for this?

I would appreciate anyone's input.
