My family member had initial ttg iga >250 with 15 or above being positive. The EMA was not done due to lack of sample. Dx with celiac disease and went gluten free.  9 month labs were just done and the ttg iga dropped to 25 and EMA was negative. Any thoughts on the negative test? Thank you.

One has to be eating gluten for the body to make the anti-gluten antibodies tested for with both of those tests. 

When one stops eating gluten for months, the body is no longer provoked to make anti-gluten antibodies, so the antibody levels go down.  

The anti-gluten antibodies in the tTg IgA decreased.  It's not surprising the other test, the EMA, is negative now since the provocative agent gluten has been removed.   

The high tTg IgA test at the beginning and the decrease on a gluten free diet, (and presumably improved health on the gluten free diet) are proof it is Celiac Disease.  

Still in denial?  Get a genetic test.  Celiac Disease is inherited.  

Having a Celiac gene shows a propensity to develop Celiac Disease, not if the gene is active or not.   But the decrease in antibodies after a gluten free diet shows the Celiac gene is active.  

One doesn't have to test positive on all the possible blood tests for Celiac Disease.  One positive test is sufficient, especially if it is over ten times the upper limit, which the prior tTg IgA test was.

Quit doubting, Thomas, and get on board the Celiac ship.

      This varies a lot from person to person. I include foods that are not certified gluten-free but are labelled "gluten-free", while super sensitive people only use certified gluten-free. Both types of products have been found to contain gluten, so there are no guarantees either way: It you are in the super sensitive group, eating a whole foods based diet where you prepare everything is the safest bet, but it's also difficult. Eating out is the the most risky, even if a restaurant has a gluten-free menu. I also include items that are naturally gluten-free, for example refried beans, tuna, pasta sauces, salsas, etc., which have a low overall risk of contamination.
