Vitamins Eg B12

Hi again,

Im after some advice mainly. Ive been gluten free for well over a month now and lactose free for over a week. The feeling sick has eased. Still not been able to gain weight, have tried putting weight on with muscle eg doing weights etc, still not change.

Now to the point lately I have noticed the tip of my tongue stings/sore etc and my mum has said it looks red (I dont know what is should look like). This isnt the first time, this was the same maybe 3 months or so ago. My mum believes I have a deficieny eg vitamin b12. I have had my blood checked recently and have been told I dont have anemia nor do I have low iron/ferratin levels. The blood test taken was a full blood count. Would this have shown if I was b12 deficient.

Also this past week or so Ive been feeling very anxious eg panic attacks, where my breathing gets all out of sync, then I feel sick and have tight chest (which could be my asthma). I feel so tense in my chest etc.

Would a b12 deficiency cause this. I dont know whether to see my gp again considering they have already said my blood was normal.

If you don't already have a copy of your test results, I'd suggest asking your doctor for them. Many times, they will say "normal" even if the level is on the low end. Also, there are certain things which make it difficult to get an accurate serum B12 measurement. There is no known level of overdose for B12, so it would not hurt to try a B12 supplement, to see how you feel. I'd recommend the methylcobalamin form, in a sublingual tablet/lozenge. A dosage of 1-5mg per day can be very helpful. A co-enzyme B-complex can also be helpful.

However, I'm not sure the stinging/sore tongue is indicative of B12 deficiency. Maybe, or maybe not, but in my experience, that is what happens when I'm dehydrated. It may also account for the redness. So I'd suggest drinking more water each day. Avoid diuretics including caffeine. It may also help to get a potassium supplement, as potassium is an important electrolyte. Coconut water is a very good source of potassium. Some say it is better than a sports drink for re-hydration. Removing dairy from the diet also reduces potassium intake, if you don't compensate with some other source(s).

A full blood count does not check for nutrient deficiencies - these have to be individually ordered. You need to know your levels of Vits. D, B12, folate, A, E, K, also potassium, calcium, zinc, magnesium. A check of your thyroid levels would also be in order.

As an aside, I think it is a bit soon to be concerned about not gaining weight - that will happen as you heal and start absorbing your food better. It does not happen overnight

I have nothing to offer on why your tongue is doing what it is; peoples' symptoms are so individual and varied.

      Not necessarily. The "Gluten Free" label means not more than 20ppm of gluten in the product which is often not enough for super sensitive celiacs. You would need to be looking for "Certified Gluten Free" (GFCO endorsed) which means no more than 10ppm of gluten. Having said that, "Gluten Free" doesn't mean that there will necessarily be more gluten than "Certified Gluten" in any given batch run. It just means there could be.

      I think it is wise to seek a second opinion from a GI doc and to go on a gluten free diet in the meantime. The GI doc may look at all the evidence, including the biopsy report, and conclude you don't need anything else to reach a dx of celiac disease and so, there would be no need for a gluten challenge. But if the GI doc does want to do more testing, you can worry about the gluten challenge at that time. But between now and the time of the appointment, if your symptoms improve on a gluten free diet, that is more evidence. Just keep in mind that if a gluten challenge is called for, the bare minimum challenge length is two weeks of the daily consumption of at least 10g of gluten, which is about the amount found in 4-6 slices of wheat bread. But, I would count on giving it four weeks to be sure.

      thank you both for the insights. i agree, im going to back off on dairy and try sucraid. thanks for the tip about protein powder, i will look for whey protein powder/drinks!   i don’t understand why my doctor refused to order it either. so i’ve decided i’m not going to her again, and i’m going to get a second opinion with a GI recommended to me by someone with celiac. unfortunately my first appointment isn’t until February 17th. do you think i should go gluten free now or wait until after i meet with the new doctor? i’m torn about what i should do, i dont know if she is going to want to repeat the endoscopy, and i know ill have to be eating gluten to have a positive biopsy. i could always do the gluten challenge on the other hand if she does want to repeat the biopsy.    thanks again, i appreciate the support here. i’ve learned a lot from these boards. i dont know anyone in real life with celiac.

      Let me suggest an adjustment to your terminology. "Celiac disease" and "gluten intolerance" are the same. The other gluten disorder you refer to is NCGS (Non Celiac Gluten Sensitivity) which is often referred to as being "gluten sensitive". Having said that, the reality is there is still much inconsistency in how people use these terms. Since celiac disease does damage to the small bowel lining it often results in nutritional deficiencies such as anemia. NCGS does not damage the small bowel lining so your history of anemia may suggest you have celiac disease as opposed to NCGS. But either way, a gluten-free diet is in order. NCGS can cause bodily damage in other ways, particularly to neurological systems.
