  12. Is there a reference range anywhere? Usually there is some sort of a reference range so you can see where that falls compared to the reference range.

Most LLMDs don't contract with ANY insurance companies, so you generally have to pay up front and then, depending on your insurance, you get reimbursed at the "out of network" rate.   20. I think it can vary, but I don't think some of them last long at all...a few days I think. Not 100% sure on this though. I just remembering people on LN saying that people should have it checked right away or take pictures (in fact that's not a bad idea even if it IS checked right away) if someone couldn't get in to see a doctor right away.
