Need Advice On 10Yr Dd Results Celiac Mom

Daughter has been having tummy pain past since Jan along with no appetite and bowel issues. Bloodwork came back and need some insight on how to proceed. I was diagnosed via biopsy 3yrs ago after a lifetime of issues Marsh IIIc. Trying to make sure she doesn't suffer as I did. I realize her results don't necessarily point directly at Celiac. Should I accept functional abdominal pain and move on or investiagte further?

End IGA AB Negative

TTG IGA 1.2 (<4)

 

TTG IGG 7 (<6)

MCV Low 79.4 (81.0-99.0)

That doesnt seem to be the whole panel to me. I could be wrong though. So, I would investigate further.

As shadowicewolf said, you are missing tests. The deaminated gliadin peptide tests are the newest, are very reliable tests for kids, and highly specific to celiac disease. You might want to request those (DGP IgA and DGP IgG). The anti-gliadin antibody test is an older test, but you might want to request that too (AGA IgA and AGA IgG).

 

Her tTG IGG is close to 20% above the normal. That's not insignificant, but as you know, the tissue transglutimase tests can indicate other problems. Are there any suspicious infections or other autoimmune diseases suspected (like hashimoto's)?

 

I would not accept functional abdominal pain as a diagnosis if you can help it - that's what I was told as a child and I went with that for the next 30 odd years.  I could have felt better so much sooner... pain isn't normal so don't let it go.

 

You could also request a biopsy for further proof. More proof would be her reaction to a few months on the gluten-free diet - if she does well then it is definitely celiac disease.

Thanks to both of you for your advice. Basically her "tummy" has been bothersome 5 to 6 days out of 7 the last four months.  She has started waking up at night in pain and is missing school here and there as well. Her pain doesn't seem to be school related either she is just as likely to be sickly on the weekends. Recently she has added fatigue and muscle aches to her complaints. We go back to the GI next Friday. Hopefully they will have some ideas or accept mine.

Pain is not a diagnosis, it is a symptom.   Putting the word "functional abdominal" in front of it doesn't make it less so.  We have pain for a reason - to make us stop doing what we are doing, that is harmful.  If we were back in the stone age, and the grass seeds we were eating caused pain, we'd stop eating them.  No different now other than the societal and cultural pressure to blend in.

      What are your daily meals? Guilty pleasure snacks? Protein bars? I feel when looking for gluten free foods they are filled with sugar cholesterol. Looking for healthy gluten-free protein bars. Something to fill since sometimes I feel like not to eat anything. Especially if on vacation and unsure of cross contamination I figure go with a salad and protein bar to fill and play it safe.

      Unfortunately, there is presently no test for NCGS. Celiac disease must first be ruled out. NCGS is thought to be much more common than celiac disease. We know that celiac disease is an autoimmune disorder but the mechanism of NCGS is less clear. Both call for an elimination of gluten from the diet.

      Thanks for your reply Trents…most appreciated.  I am unfamiliar with celiac labs terminology so I wanted to know if the presence of HLA variants (DA:101, DA:105, DQB1:0301 and DQB1:0501) that the labs detected had any merit in predisposing one to be more sensitive to gluten/carbs than the general population?  Also,  I found what you said about NCGS very interesting and I appreciate you mentioning that.  I’ve worked hard to research and advocate for myself with my Hematologist and now with a new GI, since my bowel surgery and to maintain my Vitamin B12 health concurrent with keeping my levels of Iron in the optimal range. I’ve been tested for SIBO (do not have it), biopsy showed negative for HPylori, and have had Fecal studies done (nothing showed up) and I understand how a loss of a large amount of bowel could be highly impacting re: SIBO, malabsorption and motility issues. So I’ve managed pretty well diet and elimination-wise until just recently. That said, this new problem with extreme bloating, distention and upper girth, NAFLD just occured over the last 4 months so it is new for me and I thought celiac might be a possible issue. I’ll probably just continue on in this less gluten/carbs seem to be better for me and see how reintroducing certain foods go.  Thanks again.

      Thanks bumped it up and now take all 3 vitamins 2 capsules each with the super b complex at breakfast. I will give it some time to see if I notice a difference. I am going to track my eating daily diary on a myfitness pal app to see if the "claimed" gluten free foods bother me or not.

      Please can anyone help. I was diagnosed on 23rd December and I am trying my best to get my head around all the things to look out for. I have read that yeast extract is not to be eaten by coeliacs. Why? And is this all yeast extract. Or is this information wrong. Thanks.
