24 Hour Urine/follow Up Labs

Apparently my creatinine clearance is sky high and my proteinuria is high.  I don't have the numbers yet.  My doc is still waiting on my blood work.  My blood work was completely clean sans my deamidated gliadin as recently as January of this year so I don't my blood work is going to reveal a whole lot.

 

I was referred to a urologist last year and did a pretty full workup for my prostatitis at that time.  Everything was fine from a cyst-retro/PSA perspective.  I have never had a 24 hour urine though and asked for one from my PCP last week.  

 

My PCP is waiting for my blood work to come in and compliment my results.

 

I have a little fat on my liver, Absent IGA, some bone/joint aching, and pinker than my usual for the eyes.  Those are my current symptoms.  No noticeable swollen lymph nodes, no edema, no swelling of my joints, no low back pain...

 

Not sure what to make of this.  Trying to figure out I'm going to incorporate a kidney friendly diet into my gluten-free and still maintain weight.

Is there a reason your looking at a kidney friendly diet?  You said you are still waiting for test results from your PCP.  The prostrate tests look good.  Are you concerned about a fatty liver?  I'm not quite understanding what your asking.  If your willing, give a little more information about your specific concerns.

I am not sure if this is a good place for this, but this is the third time, that in a check-up, my son, who is now 13, has to give a second sample.  He had keytones? in his urine yesterday.  It happened last year as well and the year before.  So this is becoming routine.  The number was low, so they want to check him again, same thing they said before.  Before the second samples were good.  So, what is going on here, is this a red flag to anyone?  And if so for what?  It is a red flag to me, but I have not a clue to what, just that it should not be ignored, year after year.

Frieze,  this occurs in Type1 Diabetes.  There are also other reasons for having Ketones. 

 

Survivormom, I assume the Doctor has ruled out Diabetes?

I will check that page out Colleen, but to answer your question, no not yet.  So far he passes on the second urine test.  I think its because it is usually later in the day.  He always has had a good breakfast before we go, but that is all, breakfast.   I like morning appts.  The return visits for the second test have been in the afternoon, works in the schedule better, so I see a pattern.  Just wondering why the doctor has not and if I should point it out or if I just need to let him do his job.  We all know when it comes to what gluten does to our bodies the doctors make mistakes, well, can something be being missed here too?  My house is gluten-free for me and 2 other sons, so for the past year, he only consumes gluten when he is out with friends.

That explains why he has NO fat.  We do not really follow any diet except totally gluten-free in the house.  If he is outside of the house as a party he may eat something gluten-free.  I try to go paleo, but do not force it on the family, just the gluten-free is forced on the whole house.  He eats well, plenty of meat, and veggies at dinner, lunch is 2 PBJs on gluten-free bread (UDI) and breakfast is an egg, I put bacon and gluten-free toast out for all so I am never sure how much he gets of that, but he is a one-two egg eater.  

 

Thanks for the insight, you see the same result over and over and you just wonder should the doctor want to test something else...

I guess I am curious if anyone else had experience with proteinuria and increased creatinine clearance levels.  Both numbers were high for me on the 24 hour urine test.  I do have a high protein diet and that week I did go on two endurance bicycle rides (relative to me) for the first time in quite awhile.  I've also been highly stressed from work.  From what I can gather those can be potential reasons for the increased creat but the protein is stress on the kidneys.  I didn't know if there is something I can do to get the proteinuria number down.  I think the creat can correct itself.

 

9 months ago all of my Mayo labs were within the reference ranges with the exception of my deamidated gliadin.  Surely, my blood serums haven't went from normal to tanking in just 8 months?  My creat clearance was 155 for 24 hour urine and my protein was 146.  I don't have any swelling around my ankles.  I have some intermittent bone aches and joint pains but nothing that I am medicated for.  I'm not short of breath.  So, I'm concerned about my kidney health but as far as daily life goes, I'm fully functional and took my bicycle for a 19 miler up and down hills non-stop the other day.  So "I think" I'm healthy overall considering but was taken back by my 24 hour urine metrics.

Surviormom,  because the ketones release in the first drops of urine the morning test is showing it.  It's not surprising that the second test is then clear.  I would follow up with the doctor and point out the difference in tests knowing that the first test in the morning is showing the ketones.  I would want to know why if it were me.

As you probably read, your protein number was very high.  The protein from your blood could be leaking into the urine because the kidney filters may be damaged.  Did your Doctor say he would follow up with a UACR test?  Also, in early kidney disease you may not have any symptoms ie: swelling.  And since your tests 9 months ago were fine, it would be early in the process.  The creatinine number being high would go hand in hand with the high proteninuria number.  I think additional testing would be something to consider.  Here is some good information for you to read:

If it is the start of declining kidney function, I don't think a change in diet will suffice.

My blood serum results came back spot on.  My deamidated gliadin plummeted from 83 last time to 15!  My GFR, blood calcium, potassium, creat, etc., were perfect.  

 

I'm going to stick with some of my halftime adjustments to diet and keep plowing forward.

Clear again.  I pointed out that this was the third time this has happened and the time of day as well as his eating habits.  The answers were just little pats on the head, and trust us, answers.  Basically, he had not had enough to drink to flush it out of his system yet.  No explanation as to why it was there in the first place.  Do we all have them there in the morning?  NO.  And it was explained to me that their testing is just simple in office testing its not a serious test, it is to catch a real problem, no problem here.  I am concerned that there is.  I obviously have had health problems my whole life that doctors ignored because it did not look like a serious problem to them.  Where do I take him from here if I want this looked at further?

      Hi Christiana, Many thanks for your response.  Interestingly, I too cannot eat wheat in France without feeling effects (much less than in the US, but won't indulge nonetheless).  I also understand children are screened for celiac in Italy prior to starting their education. Wise idea as it seems my grandson has the beginning symptoms (several celiacs in his dad's family), but parents continue to think he's just being difficult.  Argh.  There's a test I took that diagnosed gluten sensitivity in 2014 via Entero Labs, and am planning on having done again.  Truth be told, I'm hoping it's the bromine/additives/preservatives as I miss breads and pastas terribly when home here in the states!  Be well and here's to our guts healing ❤️

      Lol that’s so true! Hope you get clarity, it’s tough when there’s doubt. There’s so much known about celiac disease with all the scientific research that’s been done so far yet practically and clinically there’s also so much unknown, still. Out of curiosity what’s her dairy consumption like? Even compared to early years to now? Has that changed? Calcium is dependent in the mechanism of antigen presenting cells in the gut. High calcium foods with gluten grains can initiate inflammation greater.  This is why breakfast cereals and milk combo long term can be a ticking time bomb for genetically susceptible celiacs (not a scientific statement by any means but my current personal opinion based on reasoning at present). Milk and wheat are the top culprits for food sensitivity. Especially in childhood. There are also patient cases of antibodies normalising in celiac children who had milk protein intolerance/ delayed type allergy. Some asymptomatic. There were a couple of cases of suspected celiacs that turned out to have milk protein intolerance that normalised antibodies on a gluten containing diet. Then there were others that only normalised antibodies once gluten and milk was eliminated. Milk kept the antibodies positive. Celiac disease is complicated to say the least.

      And thank you for your encouragement. I am glad that her body is doing a good job fighting it. I also just want clarity for her moving forwards. She was only 6 for the last round of testing and she's 10 now, so I'm also hoping that makes a difference. It was weird during her last round of testing though, because right before her biopsy, we'd upped her gluten intake by giving her biscuits made from straight up vital wheat gluten, and her labs actually normalized slightly (lower ttg and her ema went negative). Bodies just do weird things sometimes! lol

      The first negative biopsy in 2021 just said "no pathological change" for all the samples, and the second one in 2022 said "Duodenal mucosa with mild reactive change (focal foveolar metaplasia) and preserved villous architecture." So I think Marsh score 0 in both cases, though it's not actually written in the pathology reports. I'm really hoping to get a clear positive result this time, just for her sake.

      Hopefully the biopsy gives a conclusive and correct diagnosis for your daughter. Im in the UK and have been in the situation a few years ago of trying to rule celiac in or out after inconclusive results. Many symptoms pointing to it including the classic symptoms and weight loss and folate and iron deficiency. You have to play a waiting game. I also had the label of IBS and likely food allergy. Genetic test showed low risk for celiac but not no risk. It sounds like the Gastroenterologist is on it and hopefully will diagnose what it is correctly. Food hypersensitivity (allergy) can also cause similar symptoms and inflammation as well as mimicking IBS. Milk / dairy and wheat (cereal grains) being the biggest culprits. The “oesophagitis” and “gastritis” you mentioned can be caused by another gastrointestinal disorder called “eosinophilic gastrointestinal disorders”. These are named depending on which part of the gastrointestinal tract is affected. For example eosinophilic oesophagitis, eosinophilic gastritis, eosinophilic gastroenteritis, and more rare eosinophilic colitis. They are antigen (allergen) driven. When the blood test measuring anti-ttg antibodies is positive in absence of a positive ema test - which is more specific to celiac, this can also suggest food hypersensitivity (allergy). Usually delayed type allergy similar to celiac but not autoimmune if that makes sense. In this case the ttg antibodies are transient. Which happens. I’ve first hand experience. For info, evidence of villous atrophy too can be caused by food hypersensitivity. Not just by celiac disease. In Egid disorders the six food elimination diet, under a dietitian and gastroenterologist care, is the dietary protocol to figure out the culprit or culprits. Sometimes only two food elimination diet is used at first. The number one culprit is milk protein / dairy. Followed by wheat, eggs, soy, fish and seafood, and nuts. Most are only reactive to one food group or two. Most are only reactive to milk. Hope this is a helpful reply.
