Anyone else only tested positive for Anti DGP-IgA? Is it normal to have levels this high?

Hi guys,

I got my blood panel results back a few days ago, it tested for the following:

1. Anti-tTG IgA - 1 ULN (negative)

2. GAF-3X IgA - 3 ULN (negative)

3. Total IgA - 62 ULN (negative)

4. Anti-DGP IgA - 118 U/mL (positive) - normal range 0-30 U/mL

this is the only panel I can get in my country (most hospitals have no tests for celiac at all, and this is a special lab outside of town that can test for celiac)

as you can see, the DGP IgA is extremely positive, 4x higher than upper normal range. My ANA has also been positive (2 tests this year) both 1:160X DFS/HOMOGENOUS and my sister has had celiac disease for many many years

a long with my symptoms (weight loss, various GI symptoms, peripheral neuropathy, fatigue etc etc) I'm pretty sure my chances of having celiac are very high, the rheumatologist already tested for other AI conditions and all came back negative (lupus, sjogrens etc)

has anyone had positive numbers as high or higher than this? Or results similar to mine

"Elevated antibodies against DGP are often seen in patients who have celiac disease and are on a gluten-containing diet. This is a newer test that may give a positive result in some individuals with celiac disease who are anti-tTG negative, including children younger than two years." https://www.aafp.org/afp/2017/0601/p726.html

The next logical step would be an endoscopy with biopsy of the small bowel lining to look for damage to the "villi" that is characteristic of celiac disease. Occasionally however, people who do have celiac disease have it manifested in a skin rash called dermatitis herpetiformis (aka, DH) without villi damage. This is relatively uncommon, however, and the endoscopy/biopsy is considered the gold standard test for celiac disease.

Welcome to the forum, @Whyz! By "half way to being diagnosed" I assume you mean you have had the blood antibody testing done but not the gastroscopy with biopsy. Is this correct? Were the results of your blood work positive for celiac disease?

Three days of no gluten is not likely to have much impact on serum antibody test results. I have more concern over exactly what test or tests were ordered. When you get the results back, please post them including the reference ranges for the tests for negative vs. positive. What country are you in? Do you have much choice in what doctors you see? Sounds like it might be a good idea to seek out another physician who knows what they are doing in this area of disease diagnosis. At any rate, you have the link I shared above outlining the various tests that can be run so that might be a resource you could share with a physician.

I was just thinking this same thing - requesting repeat testing prior to giving up gluten. Makes sense. Thanks!
