Eating Gluten Before The Biopsy

My biopsy should be at about the end of January, and so I'm supposed to be eating gluten until then. I've been off it for about 2 months so far, and want to stay off through christmas, just because I want to be happy! But here are some questions:

how much gluten is enough to have an effect?

how can I limit the other effects than the villi being damaged?

Gluten often makes me constipated, but then feel "poisoned" and then get diarrhea/sickness at about 4:30 in the morning. How can I avoid this? It has been a real relief to sleep through the night for the last month or so

Should I take any vitamins? The consultant mentioned B12?

Eating gluten gives me brain fog and a sort of manic, unfocused energy at the same time. Any advice on relaxing?

Also, what happens at the biopsy? The doctor today said they would be looking "up one end and down the other" . This sounds hideous.

I have read everywhere from the equilivent of one slice of bread, up to four slices of bread are needed daily for the gluten challenge, and that it should be for 6 weeks to ideally 3 months prior to testing. I don't know what is truly the minimum you would need to ingest, maybe somewhere inbetween.

I also had the symptom of awakening at 4:30am extremely ill. I had terrible stomach pain and would throw up. But even before diagnosis, I found on my own that I didn't have that experience in the morning if I eliminated whole wheat. I gave up triskets, shredded wheat, rye crisps, and switched to white bread. I was still having diarrhea and many of the other symptoms, but at least I did not have the violent morning symptoms.

You might be able to find what makes you react the worst and try to avoid that item, while still consuming fairly significant amounts of gluten.

The colonoscopy prep is the worse part of both procedures. You will have to take something to cause your colon to empty completely, and you will be on a clear liquid diet the day before the procedure and nothing to eat or drink the morning of the procedure. The EGD does not require any prep except for nothing to eat or drink overnight prior to the scope. They generally sedate you for the procedure, and you likely won't remember much of it. You might have a sore throat afterwards.

I would try to eat things I enjoyed for the period of time, at least try to make it as enjoyable as you can, even knowing you are going to be making yourself sick. Good luck, have a donut for me.

If your biopsy is the end of January you really need to be eating gluten now. The chance of false negative is high with that short of a challenge.

That said I wish I could give you some tricks to make the challenge easier. Perhaps others will have some hints.

The one thing I do want to say is that you should keep track of your symptoms from the day you start eating gluten again. Keep a detailed record. If you should become violently ill contact the doctor immediately and talk to him directly about the effects. The gluten challenge in itself is a valid diagnostic tool and for some of us can make us extremely ill. I was one of those.

Don't worry about the endoscopy. I had it and my DD had it at age 14 and it was no sweat. I was scared but I didn't even know they had done it or that I had been under. It was over and I was asking when the dr would come!! I didn't have a sore throat, my DD was just a little unsteady on her feet from the drugs but then we went to the mall for lunch!

I actually did eat some gluten yesterday (licorice. I missed it! I did know what to expect, and was just testing... afterward went for Indian food knowing there was a high chance of being cc). Anyway, I was up at 4:30, threw up, D for a few hours and then I went to work. How can I do this for a month? My head is so much clearer being off gluten too. I can read a book or watch a film ... before my concentration levels were non existent. The other thing is, I'm self employed, with no help if I'm sick. I haven't got money in the bank to cover for that.

I do know I should be eating substantial amounts of gluten every day, but I'm visiting my boyfriend's family for christmas. I cannot bear to think of the toilet shame... Besides, his mum has already started to plan me gluten-free alternatives.

Thanks for the advice about white bread. I have realized too that any kind of fibre upsets things, even well cooked vegetables and fresh fruit. When I go back to the gluten, I'm going to have to be deliberately "unhealthy" for a while! I've only been gluten free for about 2 months so far, so I hope the damage shows OK.

      learned I had a high PTH level in 2022 suspected to be due to low vit D  got my vit D level up a bit but still have high PTH   I am 70 yrs old (today in fact) I am looking for someone who also has hyperparathyroidism that might be caused by malabsorption

      I am on day 13 of eating gluten  and have decided to have the celiac panel done tomorrow instead of Wed. (and instead of extending it a few more weeks) because I am SO incredibly sick. I have almost no appetite and am not able to consume the required daily intake of calcium to try to keep up with the loss of calcium from the high parathyroid hormone and/or the renal calcium leak.    I have spent the past 15 years working hard to improve my health. I lost 50lbs, got off handfuls of medications, lowered my cholesterol to enviable levels, and in spite of having end stage osteoarthritis in both knees, with a good diet and keeping active I have NO pain in those joints- til now.  Almost all of my joints hurt now I feel like someone has repeatedly punched me all over my torso- even my ribs hurt- I have nausea, gas, bloating, headache, mood swings, irritability, horrid flatulence (afraid to leave the house or be in any enclosed spaces with other people- the smell would knock them off their feet) I was so sure that I wanted a firm diagnosis but now- I'm asking myself is THIS worth it? esp over the past 2 yrs I have been feeling better and better the more I adjusted my diet to exclude highly refined grains and processed foods. I didn't purposely avoid gluten, but it just happened that not eating gluten has made me feel better.   I don't know what I would have to gain by getting a definitive diagnosis. I think possibly the only advantage to a DX would be that I could insist on gluten-free foods in settings where I am unable to have access to foods of my choice (hospital, rehab, nursing home)  and maybe having a medical reason to see a dietician?   please let me know if it's reasonable to just go back to the way I was eating.  Actually I do plan to buy certified gluten-free oats as that is the only grain I consume (and really like) so there will be some minor tweaks I hope and pray that I heal quickly from any possible damage that may have been done from 13 days of eating gluten.

      So I've been dealing with chasing the name celiac because of my body actively dealing with health issues related to celiac though not eating. Diagnosed in 1994 before foods eliminated from diet. After 25 years with former pcp I googled celiac specialist and she wasn't because of what ive been through. I wanted my results to be sent to my pcp but nothing was sent.I have email copies.I did one zoom call with np with team member from celiac specialist in Nov 2025 and she asked me why I wanted to know why I wanted the celiac diagnosis so bad, I sad I don't, its my life and I need revalidaion because its affecting me.KB stated well it shows you are.I asked then why am I going through all this.I was labeled unruly. Its been a celiac circus and medical has caused anxiety and depression no fault to my own other than being born with bad genetics. How is it legal for medical professionals to gaslight patients that are with an ailment coming for help to be downplayed? KB put in my records that she personally spent 120min with me and I think the zoom call was discussing celiac 80 min ONE ZOOM call.SHE is responsible for not explaining to my pcp about celiac disease am I right?

      I've noticed with my age and menopause my smell for bread gives me severe migraines and I know this.Its alarming that there are all these fabulous bakeries, sandwich places pizza places popping up in confined areas.Just the other day I suffered a migraine after I got done with my mri when a guy with a brown paper bag walk in front of me and I smelled that fresh dough bread with tuna, I got a migraine when we got home.I hate im that sensitive. Its alarming these places are popping up in airports as well.I just saw on the news that the airport ( can't remember which  one)was going to have a fabulous smelling bakery. Not for sensitive celiacs, this can alter their health during their travel which isn't safe. More awareness really NEEDS to be promoted, so much more than just a food consumption!FYI I did write to Stanislaus to let them know my thoughts on the medical field not knowing much about celiac and how it affects one.I also did message my gi the 3 specialist names that was given on previous post on questions on celiac. I pray its not on deaf door.
