Hi,

I'm living in Thailand and looking after abandoned Burmese children. One baby in our care doesn't put on weight properly and since he eats solids and we tried all kind of baby cereals and other food as high caloric as possble, with no success. The child is eating 2-3 big plates of nutrious and balanced food and is wasting away in front of our eyes. I took him to the pediatric OPD and he got the diagnosis of "Nutrional Marasmus" and my questions what causes it stay unanswered as well as what I should feed him.

He has nearly all symptoms of celiac disease, so I tried to give him gluten free food and his bloated belly is gone, he can sleep, can pass stool without pain and his skin rash is gone as well. This seems to support my suspicions, but what can I do that this child puts weight on! He got last week diarrhoea and bronchtitis with fever and lost nearly one kg!

I need advice what to cook for him that he puts on some weight fast.

I cook from scratch for him, rice and potatoes and meat and vegetables plus bananas. The doctor advised me to give him 100% cow milk instead of formula. He gets multivitamins and a medicament against metabolic Acidosis.

I don't know much about this. If he still has stomach problems, a lot of people also have problems with milk. Don't take him off it unless he still has problems. Medications and vitamins can have gluten. If they are American made, I would be happy to try to call the companies for you. I know that is hard from where you live (time differences, long distance).

I bet that when he starts feeling better and playing more, he will want to eat more. If you can get an extra meal into him, like a snack, that might help.

Formula has a higher nutritional/caloric value than cow's milk I thought so I'm not sure that doctor is right. I would check the formula though seeing as I've heard of issues with formula not being gluten free. If he is over a year old and it is available in your area you might want to try some Pediasure which is a high calorie drink that is supposed to help fill nutritional voids in diet. Also, focus on high protein & high fat foods: beans, meats, oils, eggs, avocados, butter, etc. Literally add oil & butter to everything.

And I want to say bless you for doing what you are doing!

      There are other Celiac genes. HLA DQ 2 and HLA DQ 8 show up in people from Northern European descent.   People of Mediterranean descent have HLA DQ 7.  People of Asian descent have HLA DQ 9.   There's other Indigenous populations that have other HLA genes that code for Celiac disease.   Are you still having symptoms?   What do you include in your diet?  Are you vegetarian? Are you taking any prescription medication?  Omeprazole?  Metformin?   Do you have anemia?  Thyroid problems? Are you taking any vitamins or herbal supplements?

      There are eight essential B vitamins.  They are all water soluble.  Any excess of B vitamins is easily excreted by the kidneys.   Thiamine is Vitamin B 1.  Thiamine is safe and nontoxic even in high doses.  Benfotiamine and TTFD are forms of Thiamine that the body can utilize very easily.   The form of Thiamine in the supplements you mentioned is Thiamine Mononitrate, a form that the body does not absorb well and does not utilize well.  Only about thirty percent of the amount on the label is actually absorbed in the small intestine.  Less than that can actually be used by the body.  Manufacturers add thiamine mononitrate to their products because it's cheap and shelf-stable.  Thiamine and other B vitamins break down when exposed to light and heat and over time.  Thiamine Mononitrate is a form that does not break down over time sitting on a shelf waiting for someone to buy them.  What makes Thiamine Mononitrate shelf stable makes it difficult for the body to turn into a useable form.  In fact, it takes more thiamine to turn it into a useable form.   Gastrointestinal Beriberi is a localized shortage of Thiamine in the gastrointestinal tract.  High carbohydrate meals can result in gastrointestinal symptoms of Gastric Beriberi.  Fiber is a type of carbohydrate.  So, high fiber/carbohydrate snacks could trigger Gastric Beriberi.   Since blood tests for Thiamine and other B vitamins are so inaccurate, the World Health Organization recommends trying Thiamine and looking for health improvement because it's safe and nontoxic.

      Thanks very interesting I have to see if I should take these 2 vitamins along with my multi and super Vit B complex or if its too much or would hurt me. I don't have any other health issues but would love to see if this improves anything especially to feel stronger build muscle.
