Capsule Endoscopy

Good evening just another question. I have had a negative biopsy and bloods are within range. Had a capsule endoscopy which pointed out one veins that may be bleeding and some lymphangiectasia cysts. I asked the question if I could have celiac disease but further into the intestine. They said nothing was guaranteed in life expect for taxes be death. Would the capsule have shown any inflammation or flattening of the villi. I have been off gluten for eight weeks and not really knowing what to expect. I have put some weight back on and my calcium level is 2.35 should between 2.1 and 2.6. Any advice greatly welcomed.

You mentioned that you've been gluten-free, were you eating gluten daily before your celiac disease blood tests and the capsule endoscopy? If not, you could have false negative results.

Also, capsule endoscopy is not as accurate as a traditional endoscopy to diagnose celiac disease, and the study below found "The team's results show that small bowel capsule endoscopy can spot active disease with over 87% sensitivity, and 89% specificity."

I had not stopped eating gluten at the time but I had not been eating very much at all. I became anemic, hair falling out etc. had occult blood in stool had an iron infusion etc. this was back in March. Had bloods last week and told all ok. Have vitamin d tablets and I have also bought a multi vitamin supplement. I had loose atools for years put down to anxiety etc. I am eating much better now fresh veg, fruit, fish chicken. I am afraid I have caused irreversible damage to myself.

So you've gone gluten-free and are seeing symptom improvement? If so this can also indicate that you do in fact have celiac disease, or at the very least, gluten sensitivity. 

Approximately 10x more people have non-celiac gluten sensitivity than have celiac disease, but there isn’t yet a test for NCGS. If your symptoms go away on a gluten-free diet it would likely signal NCGS.

      Hi @Atl222 As @trents points out, there could be many reasons for this biopsy result.  I am interested to know, is your gastroenterologist concerned?  Also, are your blood tests showing steady improvement over the years? I remember when I had my last biopsy, several years after diagnosis, mine came back with with raised lymphocytes but no villous damage, too! In my own case, my consultant wasn't remotely concerned - in fact, he said I might still get this result even if all I ever did was eat nothing but rice and water.   My coeliac blood tests were still steadily improving, albeit slowly, which was reassuring.

      Welcome to the celiac.com community, @Atl222! Yes, your increased lymphocytes could be in response to oats or it could possibly be cross contamination from gluten that is getting into your diet from some unexpected source but not enough to damage the villi. And I'm certain that increased lymphocytes can be caused by other things besides celiac disease or gluten/oats exposure. See attachment. But you might try eliminating oats to start with and possibly dairy for a few months and then seek another endoscopy/biopsy to see if there was a reduction in lymphocyte counts.

      This is a solid, well-reasoned approach. You’re right that “koji” by itself doesn’t indicate gluten status, and the risk really does come down to which grain is used to culture it. The fact that you directly contacted Eden Foods and received a clear statement that their koji is made from rice only, with no wheat or barley, is meaningful due diligence—especially since Eden has a long-standing reputation for transparency. While the lack of gluten labeling can understandably give pause, manufacturer confirmation like this is often what people rely on for traditionally fermented products. As always, trusting your body after trying it is reasonable, but based on the information you gathered, your conclusion makes sense.
