Strange Blood Test Results...

So to just give a quick background... I'm 19 and struggled with hashimotos thyroid disease since 16. The symptoms of the hashimotos began to subside, however Gluten Intolerant symptoms began. As someone who is majoring in Dietetics in college and studying to become an RD I knew I was at risk and saw the symptoms.

CRAZY Tired

Mood Swings

Depressed

Belly bloat (I call it my gluten belly:] )

Alien noises coming from my stomach

And EVERY gastrointestinal issue you could imagine!

I've been strict gluten-free for about 9 months. But I slip sometimes. I work in a Japanese restaurant and sometime I eat soy sauce.

ANYWAY.... I went to get a celiac panel and it was negative. Which was expected, since I'm not celiac just intolerant. But something I didn't expect in the test was that my allergic reaction numbers were through the roof. No seasonal allergies or anything. And my only other serious allergy is nuts. And I KNOW when I'm having a reaction to nuts. I can't freakin' breathe. Only allergy I can think of is the Gluten. Does this sound familiar to anyone? My doctor thinks its odd... and so do I. Can Gluten Intolerance produce allergic reaction results in a blood test?

What kind of blood test? Celiacs with low total IgA show anti-gliadin IgG.

I had a childhood wheat allergy and I seem to have lost my allergic desensitization to wheat since going gluten-free. I have allergic symptoms when I get glutened as well as the GI symptoms now.

Your blood test would be negative because you have been gluten free for 9 months. You could still be celiac. Did you tell the doctor you had been gluten free? You need to do a challenge eating at least 3 to 4 slices of bread a day for about 3 months for any chance of a correct antibody test. Even then there is a 20 to 30% chance of a false negative. If the challenge makes you ill IMHO you have your answer.

      Hi @Atl222 As @trents points out, there could be many reasons for this biopsy result.  I am interested to know, is your gastroenterologist concerned?  Also, are your blood tests showing steady improvement over the years? I remember when I had my last biopsy, several years after diagnosis, mine came back with with raised lymphocytes but no villous damage, too! In my own case, my consultant wasn't remotely concerned - in fact, he said I might still get this result even if all I ever did was eat nothing but rice and water.   My coeliac blood tests were still steadily improving, albeit slowly, which was reassuring.

      Welcome to the celiac.com community, @Atl222! Yes, your increased lymphocytes could be in response to oats or it could possibly be cross contamination from gluten that is getting into your diet from some unexpected source but not enough to damage the villi. And I'm certain that increased lymphocytes can be caused by other things besides celiac disease or gluten/oats exposure. See attachment. But you might try eliminating oats to start with and possibly dairy for a few months and then seek another endoscopy/biopsy to see if there was a reduction in lymphocyte counts.

      This is a solid, well-reasoned approach. You’re right that “koji” by itself doesn’t indicate gluten status, and the risk really does come down to which grain is used to culture it. The fact that you directly contacted Eden Foods and received a clear statement that their koji is made from rice only, with no wheat or barley, is meaningful due diligence—especially since Eden has a long-standing reputation for transparency. While the lack of gluten labeling can understandably give pause, manufacturer confirmation like this is often what people rely on for traditionally fermented products. As always, trusting your body after trying it is reasonable, but based on the information you gathered, your conclusion makes sense.
