Question About Symmetry

Hi there,

This is my first post on this forum! I am currently awaiting blood test results for Celiac. My daughter was diagnosed with it over a year ago and I've been putting off getting the screen for reasons I'm not exactly sure of - mostly because I was convinced I didn't have it. However, about 2 months ago I developed a rash. It started on my ankles, was SEVERELY, UNBEARABLY itchy, and has left purple scars. Since then, patches have appeared all over my body (but never as bad as the first ones on my ankles). My question is, if it's DH, does the rash always have to be symmetrical? Mine is loosely symmetrical - for example, it was on the front of my left ankle but the back of my right ankle, it's appeared on both legs and arms but not exactly in the same places, and while one elbow has developed it, so far, the other hasn't. It's also on the back of my neck.

In every other way, it seems to resemble what I've read about DH, so I was just wondering how exact the symmetry was supposed to be.

Your suspicions may very well be correct, as DH is not always symmetrical. But even if it isn't DH, it doesn't mean you aren't sensitive to gluten. Many members here have and do experience odd rashes, blisters, and other manifestations on the skin.

I personally don't believe enough studies have been done of DH to establish whether or not it's always symmetrical. I can only say from a personal point of view that my son and I (both celiacs) get DH on our faces and necks....and then I also get it on the sole of my right foot and on my left hand, while my son gets it on the sole of his left foot and on his right hand. Weird....but true. Obviously, the idea that DH must absolutely be symmetrical can be easily contradicted.

      Hi @Atl222 As @trents points out, there could be many reasons for this biopsy result.  I am interested to know, is your gastroenterologist concerned?  Also, are your blood tests showing steady improvement over the years? I remember when I had my last biopsy, several years after diagnosis, mine came back with with raised lymphocytes but no villous damage, too! In my own case, my consultant wasn't remotely concerned - in fact, he said I might still get this result even if all I ever did was eat nothing but rice and water.   My coeliac blood tests were still steadily improving, albeit slowly, which was reassuring.

      Welcome to the celiac.com community, @Atl222! Yes, your increased lymphocytes could be in response to oats or it could possibly be cross contamination from gluten that is getting into your diet from some unexpected source but not enough to damage the villi. And I'm certain that increased lymphocytes can be caused by other things besides celiac disease or gluten/oats exposure. See attachment. But you might try eliminating oats to start with and possibly dairy for a few months and then seek another endoscopy/biopsy to see if there was a reduction in lymphocyte counts.

      This is a solid, well-reasoned approach. You’re right that “koji” by itself doesn’t indicate gluten status, and the risk really does come down to which grain is used to culture it. The fact that you directly contacted Eden Foods and received a clear statement that their koji is made from rice only, with no wheat or barley, is meaningful due diligence—especially since Eden has a long-standing reputation for transparency. While the lack of gluten labeling can understandably give pause, manufacturer confirmation like this is often what people rely on for traditionally fermented products. As always, trusting your body after trying it is reasonable, but based on the information you gathered, your conclusion makes sense.
