Help...Feeling like I can't eat anything

Hi all, 

( Excuse any spelling, grammar I have mild dyslexia and and writing this in a rush )

For context,  I'm 41,  6ft tall,  skinny build at the high end of 11 stone,   so in the doctors eyes im healthy,  I look healthy and my only issue is I've never been able to put on weight and if i excersize I shed weight fast ( so im fairly unfit for my build )

So, I have been tested and I don't have Celiac, or crons or anything else that can pin point after having years of blood tests and recently alot more.........which is very frustrating as at least it would give me some kind of reasonaning and focus.

For 20 years ive struggled with bad guts (without sounding cras,  not diarrhea but not formed stools and bad fatigue,  but ive always eaten everything and just put up with it and tried to not let it effected my lifestyle, as ive got older, the last few years my gutts are getting worse and its been made worse with rashy forhead, around the nose,  flaky scalp and rashy between my chest if when i react to something ( Which seem to be every day ),   last 6 months its stepped it up a gear and really feeling it,   even when really narrowing my diet to the point i just dont know what to eat anymore.

Ive been through a hydrogen breath test and transit study,    I have fast transit,  and few weeks back have been confimed to be Lactose intolleratant with a reading of the breathtest being 277, which im told is significantly high,  not just high.    From my own observations of trying gluten and dairy free diets,   glutten and dairy free foods make me worse and make me go from a manageable once a day not formed stool to diarrhea and not knowing if/when i need to go so as a result ive put glutten back in my diet to go back to that once a day stablity.   I suspect with such a high reading in the breath test im not only Lactose,  but basically any kind of "tose" intolleratent,  which alot of the gluten and dairy free foods have glucose fructose syrup which ive narrowed down to be one possible or many reactions.

I also done a supplylife reactivity test,   it came back saying im basically gluten and dairy reactive to a high level,   but if i can't have the "toses" as well as gluten and dairy,  Im basically eating air.  

I've tried to go meat, fish etc only and chips and have been very strict,  cutting everything out just so i can have a few weeks of reasonable guts and no reactions,   and then start adding foods back in from there,  but I can't seem to even get to that point where im reactive free for a few weeks.

I thought some of you guys might be able to offer some advise,   as I know im not celiac,  but i feel i might be fairly close if that makes sense without a diagnosis.  

Many thanks for any tips,  I've even considered fasting but as someone who loves their food and gets hangry very quickly that seems like a last resort haha.

Have you been checked for SIBO (Small Intestine Bacterial Overgrowth) and have you considered that you might have MCAS/histamine intolerance? MCAS is Mast Cell Activation Syndrome.

Yes I was tested for SIBO in last round of tests i believe as thats what the doc thought ig mitht have been.    I've not heard of the MCAS/histamine though so will look into that and mention it when i go back next month for my next appointment.

You can ignore the gluten-free parts of this article if you don't have celiac disease or non-celiac gluten sensitivity, which it sounds like may be the case.

Some common food intolerance issues are dairy/casein, eggs, corn, oats, and soy.

Many on this forum have gone the not celiac, misdiagnosed this that and the other thing until one day 10 years later... Oh, you have Celiac Disease.

The breath test tests for lactlose intolerance measures H2 indicating lactose intolerance and Sibo Lactose Intolerance: What Your Breath Can Tell You

Adult humans do not make enough denovo lactase after childhood.  Lactobacillus excretes lactose.  It's a symbiotic relationship.  Fermented pickles.  If it has vinegar it is a quick pickle and has the sour taste but not the lactobacillus.

Start with GFD but avoid prepackaged, stick to real food.  Just because they say you don't have Celiac, does not mean you have to eat the garbage.  Point in fact, our western diet is highly inflammatory. Estimates around 14:1 omega 6 to omega 3.  Wheat is 22:1, white potato 3:1; sweet potato 14:1.  Pickle 0.74:1.   Nutrition Comparison Tool

There is a newish designation: Non Celiac Gluten Sensitivity.  Around 10% of western diet get it compared to 1% Celiac.  To diagnose first exclude Celiac Disease. Then look for improvement on GFD.  You have to give it at least a month before giving up.  After two weeks you may feel well enough so you are tempted to try some gluten.  Been there, done that. Don't do it.  Gluten is very adictive in some people. Your Hangry toxic hunger could be a symptom.

Ever notice that when you tell someone to not eat wheat they get angry at the mention?  Tell them not to eat eggs or watermelon and they are fine with that but "I'll die before I give up my bread."  Yes, they will.

Secondary to your gut issues is malabsorption syndrone, leading to malnutrition.  Western Medicine does not pay a lot of attention because, well malnutrition only happens in the third world and in alcoholics.

I was a hard core alcoholic from 1985 until 2014.  The addiction was gone after only three days gluten free.  

Because I still have anorexia here is a list of vitamins I take.

  5. Iodine for Hormonal Health "Your ovaries also need iodine and without enough their structure changes. Iodine-deficient women can produce ovarian cysts and are at risk of developing PCOS."
  6. Vitamin B2 helps break down proteins, fats, and carbohydrates. It plays a vital role in maintaining the body's energy supply. 
  7. 500 mg Nicotinic Acid vitamin B3 - increase capillary blood flow, thats the flush you get at first. 
  8. 500 mg Pantothenic Acid vitamin B5- creates energy from glucose in the Krebbs Cycle
  9. 1000 mcg B12 - creates hemoglobin for oxygen transport

  11. I've been using Zinc Glyconate (Cold-Eeze) since 2004 anytime I feel an itchy throat or other sign of air borne virus coming on.  I haven't had cold or flue, including Covid 19, since.
  12. Naturally fermented dill pickles reestablished lactobacillus in my gut and reversed my recently acquired lactose intolerance.  100% grass fed dairy is less inflammatory. Milk is a good source of iodine.  Whole milk a good source of vitamin A, calcium, magnesium, phosphorus.  Fat reduced milk has added synthetic vitamin A in the USA.  
  13. Omega-3 and Omega-6 Fatty Acids in Vegetables  Eating more of the vegetables low in omega six and high omega 3 can reduce inflammation while increasing vitamin and mineral intake..  The target omega 6:3 ratio is less than 3:1.  The typical western diet is from 14:1 to 20:1 because of our food choices.

Folate, potassium, magnesium, calcium, vitamins A and E best to get from food.

Thanks for all the replies,  some really good info.   I'm going to read it properly tomorrow but its super interesting and goes along with my own thoughts that its possible i may get a diagnosis later down the line.

      @catnapt,  I'm sorry you're having such a rough time.   How much wheat germ and how much gluten were you eating? Lectins in beans can be broken down by pressure cooking them.  Do you pressure cook your beans?  Were you pressure cooking your wheat germ? What drugs are you taking?  Some immunosuppressive drugs affect IgA production.  Do you have anemia?

      oops my gluten challenge was only 12 days It started Jan 21s and ended Feb 1st   worst 12 days of my life   Does not help that I also started on a thiazide-like drug for rule in/out renal calcium leak at the exact same time No clue if that could have been symptoms worse 🤔

      Welcome to the forum @Known1, What reaction were you expecting? Pipingrock.com High Potency Vitamin D3, 2000 IU, 250 Quick Release Softgels $6.89 I've have been taking the 10,000 IU for close to 10 years. When I started with vitamin D I worked my way up to 10000 over several weeks.  Even at 8000 I felt no noticeable difference.  Then after a few days at 10000 it hit Whoa, sunshine in a bottle.  celiac disease causes malabsorption of dietary D and you've poor UV access.  It took me from 2015 to 2019 to get my 25(OH)D just to 47 ng/ml.  Another two years to get to 80.  70 to 100 ng/ml seems to be the body's natural upper homeostasis  based on lifeguard studies.  Dr. Holick has observed the average lifeguard population usually has a vitamin D 3 level of around 100 ng/ml. Could it be that our normal range is too low given the fact that ¾ or more of the American population is vitamin D deficient? Your Calcium will increase with the vitamin D so don't supplement calcium unless you really need it.  Monitor with PTH  and 25(OH)D tests. Because of your Marsh 3 damage you need to ingest way more than the RDA of any supplement to undo your specific deficiencies. I believe you are in the goiter belt.  Unless you have reason not to, I recommend pipingrock's Liquid Iodine for price and quality.  The RDA is 150 to 1100 mcg.  In Japan the safe upper level is set at 3000 mcg.  Start with one drop 50 mcg to test for adverse response and build up.  I found 600 mcg (12 drops) a day is helping repair my body.  Iodine is necessary to healing.  90% of daily iodine intake is excreted in urine.  A Urine Iodine Concentration (UIC) can tell how much Iodine you got that day.  The thyroid TSH test will not show iodine deficiency unless it is really bad.

      I don't know if I am getting sufficient Omega Threes. I read about  phosphotidyl choline may cause heart issues. I will have o do further research on heathy Omega 3 supplements or from foods. Is there a blood test that can tell you everything level in your system such as Thiamine, Benfotiamine levels etc? Thanks

      If lectins were my problem, I would react to wheat germ (the highest source of wheat lectins) and beans. I don't. I only react to bread and pasta, which are the highest sources of gluten. Therefore, my issue is wheat-specific (Gluten/ATIs), not a general lectin issue.   I have eaten a supposedly high lectin diet (I say supposedly because lectin content in these foods is greatly reduced by proper cooking and I eat very few of those foods raw, and even then, rarely!!) for years. My health has improved greatly on my whole foods plant forward diet. I have asked all my drs and a registered dietician about my diet, asked if eating such a high amnt of fiber might interfere with the digestion of any other nutrients and the answer has always been NO.     while doing the gluten challenge I did not eat ANY wheat germ (since it doesn't have hardly any gluten, and I was too sick from the bread and pasta to want to eat much anyway) I will NOT put that poison in my body again. That was a horrific experience and if this is what most celiac patients have to deal with, I am very sorry for them I don't care if I have celiac or NCGS I won't intentionally cause myself that much pain and suffering it's not worth it.
