Two year old - blood test interpretation

Hello,

Please help me to understand the results of this test. My daughter is just 2 years old has not grown for 8 months and experiences extreme constipation. Other than that she is normal. We had blood work done and the Doctor simply referred me to a ped gastro doctor who now recommends an endoscopy and biopsy. I am trying to figure out if this is right for us. My nephew had a similar situation and his scope was inconclusive at age 5, his symptoms prevail and worsen as they wait for their next step.

Here is what I received from the blood work -

IGA, Serum 184 Mg/dL   ........standard range is 24-121  Flagged as High, Serological evidence of celiac disease is present

Tissue Transglutam AB IGA 1 U/ML.......standard Range is <4 U/mL  , Interpretation <4 : no antibody detected,  >4 antibody detected

Gliadin AB (IGA)  40 Units .................standard <20 units, Flagged as High, Interpretation <20 Antibody not detected, >20 Antibody detected

I am so confused and trying to decide if we should get the scope or just go gluten free 100% for a few months and redo blood work.

Any advise or knowledge is appreciated!

I am sorry that your baby is ill!  

Your daughter does have a positive on the DGP IgA (like me).  It often catches very young celiacs or elderly ones (like me).  Her Immunoglobulin A is high too and that can be elevated with autoimmune disorders.   I would consult with the PED GI.  Talk over your concerns about the endoscopy.  He or she may decide to diagnose based on the diet and a re-check on antibodies (should decline).  Let your GI help, you make the best decision for you and your daughter.  In the meantime, get prepared. Continue to research the disease and diet.   Keep her on gluten until all testing is complete.

      Thank you that’s really helpful, hopeful won’t have to have a biopsy.

      That means the normal range (i.e. not celiac disease) would be a result less than 14.99.  Your result is WAY above that. Some gastroenterologists would diagnose that as celiac disease even without a confirming biopsy because it is more than ten times the top of the normal range.

      Hi everyone. Back in 2022 I had blood work drawn for iga ext gliadin. Since then I’ve developed worse stomach issues and all other health issues. My doctor just said cut out gluten. He did no further testing. Please see my test results attached. I just need some direction cause I feel so ill and the stomach pain is becoming worse. Can this test show indications for other gastrointestinal diseases?

      Thank you. These were the results TTG ABS NUMERICAL: > 250.0 U/mL [< 14.99]  Really don’t understand the results!

      Clearly from what you've said the info on Dailymed is much more up to date than the other site, which hasn't been updated since 2017. The fact that some companies might be repackaging drugs does not mean the info on the ingredients is not correct.
