Eosinophilic Esophagus

So, I had my endoscopy last week. Prep was 2 weeks of gluten after a 5 month total abstinence. I started adding gluten slowly and felt pretty rough the first week, but the second week was actually easier--and I was eating a lot more gluten the second week. A complicating factor was that due to an excess of mosquito bites I was taking allegra & benedryl the second week. Perhaps that can reduce food allergy symptoms as well?

In any case, I'm waiting for the results. However, I have to say that I think Celiac is unlikely based on the pictures of my small intestine. It looks delightfully healthy, despite my best efforts to aggravate known food sensitivities. Since I was feeling better, not worse the second week, I decided to 'go for broke' and had garlic bread and pizza the night before the endoscopy. Dairy has a history of making me quite ill as well.

Curiously, among the pictures provided by my excessively techie GI guy from the tour of my gullet, were clear shots of a very swollen esophagus, described as possibly eosinophilic esophagus. Seems there are two known causes for this. One is GERD, which I have a 10-year history of, and the other is food allergies, eosinophilic esophagus.

Anyone else had this unexpected finding of a swollen esophagus? Does gluten consumption get easier for anyone else 'with practice'? Has anyone else found that taking antihistamines affects the reaction?

Thanks!

www.apfed.org is a good resource for those with eosinophilic disorders, if that ends up as your diagnosis.

I hate to be the bearer of bad news, but two weeks back on gluten (after 5 months gluten free) most likely is not enough for an accurate Celiac biopsy. My daughter's doc said two months.

That being said, I don't know why your symptoms weren't as bad the second week. If you have a (wheat?) allergy, the benedryl could have played a role. It wouldn't have with Celiac. Also, EE could indeed be your problem instead of Celiac. My daughter's GI thought she had EE and that is why she was scoped (found Celiac instead ;) ). Her primary symptom was GERD that improved but did not go away even though she was being medicated (Prevacid). If you could tell from the photos that your throat was swollen, I would hope that your doc would have biopsied the area to determine if EE is your diagnosis.

Good Luck.

Wow! I am glad that you wrote about EE - my 7 year old was diagnosed Celiac in April and EE in May (when she was scoped). Her's is food allergy (allergic to all grains, corn, legumes and more!!!) She is both celiac and EE. I have also heard that you needed to be on gluten for a longer period of time, but I am no expert. I am not sure of your age - our doctor told us that there is a period for teens that tends to make them not feel the effects of gluten (even though damage is still being done to the intestine). Maybe that is why you are feeling fine? I know that Clariton helps my daughter's throat feel better after swelling, but not sure how it works with her gluten feelings. It is hard for me to offer you information on her feelings because she never knew what it was like to not live with celiac or EE.

Got my results tonight, they found nothing. The assumption is that the swelling is from GERD. No evidence of EE or of any other issues. So that's good. Neither Celiac or EE sounded like much fun.

I can try and see if an extra dose of Prilosec fixes it. I would love it if my 'food issues' are all in my head!

I am so glad to hear about your results! Hope that you feel better soon! - Julie

I am being tested for food allergies and my doctor told me to stop taking my allergy meds (Claritin D) one week before the tests. It can mess with the results. I know that info is too late for you but it might help someone else.

Is there a digestive enzyme that helps build a healthier gut? I see people taking them but not sure what really works

So the tTG-IGA at 28 is positive for celiac disease. There are some other medical conditions that can cause elevated tTG-IGA but this is unlikely. There are some people for whom the dairy protein casein can cause this but by far the most likely cause is celiac disease. Especially when your small bowel lining is "scalloped". Your Serum IGA 01 (aka, "total IGA") at 245 mg/dl is within normal range, indicating you are not IGA deficient. But I also think it would be wise to take your doctor's advice about the sucraid diet and avoiding dairy . . . at least until you experience healing and your gut has had a chance to heal, which can take around two years. After that, you can experiment with adding dairy back in and monitor symptoms. By the way, if you want the protein afforded by dairy but need to avoid casein, you can do so with whey protein powder. Whey is the other major protein in dairy.

hi, i want to say thank you to you and @trents   . after 2 phone calls to my GI, her office called me back to tell me that a blood test was “unnecessary” and that we should “follow the gold standard” and since my biopsy did not indicate celiac, to follow the no dairy and sucraid diet. i luckily have expendable income and made an appt for the labcorp blood test that day. i just got my results back and it indicates celiac disease i think 😭   im honestly happy bc now i KNOW and i can go gluten free. and i am SO MAD at this doctor for dismissing me for a simple blood test that wouldn’t have cost her anything !!!!!!!!!!! im sorry, im so emotional right now, i have been sick my whole life and never knew why, i feel so much better already   my results from labcorp:   Celiac Ab tTG TIgA w/Rflx Test Current Result and Flag Previous Result and Date Units Reference Interval t-Transglutaminase (tTG) IgA 01 28 High U/mL 0-3 Negative 0 - 3 Weak Positive 4 - 10 Positive >10 Tissue Transglutaminase (tTG) has been identified as the endomysial antigen. Studies have demonstrated that endomysial IgA antibodies have over 99% specificity for gluten sensitive enteropathy. Immunoglobulin A, Qn, Serum 01 245 mg/dL 87-352

      Thank you this really helped.

Okay that is really good to know. So with that being positive and the other being high it makes sense she diagnosed her even without the endoscopy. So glad we caught it early. She had so many symptoms though that to me it was clear something was wrong.   yeah I think we had better test us and the other kids as well.
