Do I need to be eating gluten before skin biopsy?

Hello I'm new. I have what I'm pretty sure is DH on my elbows and shins. Intensely itchy, red blisters that when scratched look like bites. Back in Sept 2015 I had them on both knees too. I have hives most day, have a burning sensation  and feel 'poisoned' I also have ibs and fibro. 

I came off gluten in Nov and the rash became less red and the itching stopped within a week, I now just have pale brown scars. My GP wanted to test me for celiac disease and asked me to go back on the gluten for 6 weeks but I lasted 7 days. Felt terrible. That was end of December, the rash started up again early Jan and it's worse than ever. I am going to see a dermatologist in the next week or two.

 

Would he be able to get a positive for DH if I've been off gluten or do I need to go back on it again?

Yes, you have to be eating gluten for the skin biopsy the same as you do for the blood panel.

Omg! I was diagnosed with celiac in Nov. I had all the stomach problems. I ate gluten last week.. And have been having the hives ALL over. I just went to the dermatologist and took 2 biopsies in order to see if the hives are connected to DH.

i am suffering! I feel your pain. Any advice you could share with me?

katelyann, it doesn't matter if your hives are dh. You have celiac disease, no need to be dx'd with dh. You just have to be super careful not to get cross contaminated. Since your gluten exposure was assumedly only once last week then your biopsies will turn up negative for dh IF the derm took them correctly. You would have had to be eating gluten for a while for the biopsies to come up positive. You ought to take a look at hives & dh & compare because they are completely different things.

If it is not DH, then consider researching a histamine intolerance which may just be temporary.  I am not a doctor, but something to think about.  In any case, remaining gluten-free is critical.  Sorry to hear you were glutened!  :(

      Hi, I posted before about my son's legs shaking after gluten. I did end up starting him on vit b and happily he actually started sleeping better and longer.  Back to my 4 year old. She had gone back to meltdowns, early wakes, and exhaustion. We tested everything again and her ferritin was lowish again (16) and vit d was low. After a couple weeks on supplements she is cheerful, sleeping better and looks better. The red rimmed eyes and dark circles are much better.   AND her Ttg was a 3!!!!!! So, we are crushing the gluten-free diet which is great. But WHY are her iron and vit d low if she's not getting any gluten????  She's on 30mg of iron per day and also a multivitamin and vit d supplement (per her dr). That helped her feel better quickly. But will she need supplements her whole life?? Or is there some other reason she's not absorbing iron? We eat very healthy with minimal processed food. Beef maybe 1x per week but plenty of other protein including eggs daily.  She also says her tummy hurts every single morning. That was before the iron (do not likely a side effect). Is that common with celiac?

      Hi @Atl222 As @trents points out, there could be many reasons for this biopsy result.  I am interested to know, is your gastroenterologist concerned?  Also, are your blood tests showing steady improvement over the years? I remember when I had my last biopsy, several years after diagnosis, mine came back with with raised lymphocytes but no villous damage, too! In my own case, my consultant wasn't remotely concerned - in fact, he said I might still get this result even if all I ever did was eat nothing but rice and water.   My coeliac blood tests were still steadily improving, albeit slowly, which was reassuring.

      Welcome to the celiac.com community, @Atl222! Yes, your increased lymphocytes could be in response to oats or it could possibly be cross contamination from gluten that is getting into your diet from some unexpected source but not enough to damage the villi. And I'm certain that increased lymphocytes can be caused by other things besides celiac disease or gluten/oats exposure. See attachment. But you might try eliminating oats to start with and possibly dairy for a few months and then seek another endoscopy/biopsy to see if there was a reduction in lymphocyte counts.
