Vomiting Undigested Food/meds After 7 Hours!

Hi, all. I posted a few times recently, and thank you to everyone who has responded! It's been so helpful. This time I'm writing about a new sypmtom. Late last night, I ate a small meal and about an hour later took my regular fiber supplement (capsule form). I went to bed and woke up about seven hours after eating with intense epigastric pain so severe that I could barely stand up or lie straight. I spent about half an hour curled up in a ball then began vomiting. What concerns me, besides the pain, is the fact that I vomited whole peas; swollen, intact fiber capsules; and mushy peach-colored clumps (carrots?). I called my GI doc., who has diagnosed me with GERD, and he told me to just swtich my fiber supplement to one not as irritating and to stick to light meals. (I'm supposed to be on fiber for constipation). He told me this often happens with GERD patients. I'm not convinved. Shouldn't food/meds be digested after 6-7 hours in the stomach? Could this be gastroparesis and/or a symptom on celiac disease/gluten intolerance? Thanks in advance.

How fast stuff gets broken down depends on a lot of different factors. If you had said you were clearly able to identify the contents above the peas and the capsules I would be more concerned. If this starts to happen on a regular basis you really should get back to your doctor though.

I agree with him you should drop those fiber capsules. But you may want to try adding more fiber in your diet if you can but not in the form of grains. Cooked apples, sweet potatoes (an ancient chinese cure for C and D), cooked peaches etc. Look for foods high in soluable fiber rather than the fiber found in foods like celery. They should be easier to digest.

Gerd can be associated with celiac, it was one of my daughters main problems.

One last thing try not to go to bed or lay down too soon after eating while the Gerd is still an issue. I wouldn't eat for at least a couple hours before retiring and if you can elevate the head of your bed a bit with books, bricks whatever works, that can help also.

My main symptoms before being diagnosed with celiac were GERD and diarrhea. I felt as if it took food forever to leave my stomach. GERD sometimes is a lack of acid in your stomach rather than excess acid. I think that was true for me. I was on Prevacid for several years and it did not help anything--taking digestive enzymes and betaine hydrochloride (digestive acids, basically) has helped a lot.

I was glutened a few weeks ago and was, uh, going at both ends. I also noticed that I had not digested my previous meal (from 6 hours before) at all--apparently I just shut down when glutened. I'm not sure if that is relevant or TMI! Good luck.

If you were vomiting whole peas, that means that you never chewed them in the first place, which might be part of the problem.

Also, if you have an intestinal virus, then it is perfectly normal to vomit undigested food, even after several hours. That's how the body rids itself of toxins in the upper digestive tract.

GERD and constipation are very common amongst celiacs.

I agree with the above poster that you need fiber in your diet in the form of FOOD. You do not have a fiber capsule deficiency! Stone fruits (peaches, pears, plums, mangoes, etc.) are among the easiest to digest sources of fiber. You don't need to cook them; in fact, it's probably better if you don't. Just make sure that you wash them!

Perhaps you could post your typical days' diet here, and the veterans on this board might be able to offer helpful suggestions.

My daughter's only symptom was acid reflux and at the time of her Celiac diagnosis, she was also diagnosed with gastroparesis. The GI thought the gastroparesis was a direct result of Celiac and would go away once she had healed. The only time she vomitted (and food was identifiable many hours later) was when she had a stomach virus.

Now, even though my daughter was diagnosed with gastroparesis, she never really had any symptoms (other than the acid reflux). Many people with gastroparesis can't eat much before they feel overly full. They have a difficult time consuming their daily calorie requirements. Many switch to a low fiber diet to help the stomach empty more quickly.

If this is the only time it has happened and doesn't continue to happen, I would hazard a guess that it was viral. If it continues to happen, I would talk to your doctor.

I have gastroparesis and GERD. Also have food allergies but not celiac. I do not get stomach pain when my stomach is acting up. It will just feel weird. Like not quite right. And I feel like if I massage the area it will help. But it never does. Sometimes I don't even get the weird feeling, but I will feel saliva backing up in my mouth. I then know I have to get the bowl really quickly because I'm going to throw up.

In the beginning, it was very violent, sometimes lasting for hours and coming from both ends at once. The throwing up iteself hurt, but it didn't hurt before that.

I had to switch to a low fiber diet. I was trying to do a raw vegan diet and was eating huge salads daily. I can no longer do that. I felt off earlier today because I had salad for dinner two days in a row. I guess I wasn't thinking when I did that. Now my meals are usually pretty much the same, but it has to be that way. I haven't thrown up *knock wood* for a long time and I want to keep it that way. And yes, the food and meds are not digested when they come back up.

Celiac disease is the most likely cause, but here are articles about the other possible causes:

Hi @Atl222 As @trents points out, there could be many reasons for this biopsy result.  I am interested to know, is your gastroenterologist concerned?  Also, are your blood tests showing steady improvement over the years? I remember when I had my last biopsy, several years after diagnosis, mine came back with with raised lymphocytes but no villous damage, too! In my own case, my consultant wasn't remotely concerned - in fact, he said I might still get this result even if all I ever did was eat nothing but rice and water.   My coeliac blood tests were still steadily improving, albeit slowly, which was reassuring.

Welcome to the celiac.com community, @Atl222! Yes, your increased lymphocytes could be in response to oats or it could possibly be cross contamination from gluten that is getting into your diet from some unexpected source but not enough to damage the villi. And I'm certain that increased lymphocytes can be caused by other things besides celiac disease or gluten/oats exposure. See attachment. But you might try eliminating oats to start with and possibly dairy for a few months and then seek another endoscopy/biopsy to see if there was a reduction in lymphocyte counts.

This is a solid, well-reasoned approach. You’re right that “koji” by itself doesn’t indicate gluten status, and the risk really does come down to which grain is used to culture it. The fact that you directly contacted Eden Foods and received a clear statement that their koji is made from rice only, with no wheat or barley, is meaningful due diligence—especially since Eden has a long-standing reputation for transparency. While the lack of gluten labeling can understandably give pause, manufacturer confirmation like this is often what people rely on for traditionally fermented products. As always, trusting your body after trying it is reasonable, but based on the information you gathered, your conclusion makes sense.
