Are Baseline Bloodtests Helpful Later?

I DID ask about the celiac panel, I must have misunderstood...

because after a 3 month gluten challenge (following a year of gluten lite because of very low carb) my husband's blood work shows only an Endomysial which was negative and some other tests I hadn't expected:

WBC 3.2 (range 4.5 - 13)

RBC 3.74 (4.5 - 5.3)

HGB 11.8 (13.0 - 17.5)

HCT 34.3 (37.0 - 49.0)

MCV 91.6 (80.0 - 100.0)

The GI said he didn't now what could be causing these blood #'s and we should f/up with the internist. I'm pretty positive the anemia is resolving; he's much less pale, So presumably the WBC will too.

The endoscopy went ok, but it took awhile to get the results so it has been 2 weeks gluten-free.

Should he ask for more bloodwork at his next appt 11-18?

Is having a baseline helpful to see how it is going with the diet?

is this too long gluten-free for the tests to work?

If directlabs is any good, he could get those tests done sooner. He's never had any positive bloodwork. He has tested postive at Enterolab for gluten, casein, soy and (just barely) eggs.

the biopsy itself was negative, but I don't have the pathologist's report in writing yet and I'm sort of doubtful. I mean, is this doc really on the ball? He's on the list from the local CSA support group and he's very supportive of the diet despite the negative biopsy I'll put the biopsy stuff in a separate post.

Thank you very kindly for any input, suggestions

What you’re describing is actually very common, and unfortunately the timing of the biopsy likely explains the confusion. Yes, it is absolutely possible for the small intestine to heal enough in three months on a strict gluten-free diet to produce a normal or near-normal biopsy, especially when damage was mild to begin with. In contrast, celiac antibodies can stay elevated for many months or even years after gluten removal, so persistently high antibody levels alongside the celiac genes and clear nutrient deficiencies strongly point to celiac disease, even if you don’t feel symptoms. Many people with celiac are asymptomatic but still develop iron and vitamin deficiencies and silent intestinal damage. The lack of immediate symptoms makes it harder emotionally, but it doesn’t mean gluten isn’t harming you. Most specialists would consider this a case of celiac disease with a false-negative biopsy due to early healing rather than “something else,” and staying consistently gluten-free is what protects you long-term—even when your body doesn’t protest right away.

Yes, I meant if you had celiac disease but went gluten-free before screening, your results would end up false-negative. As @trents mentioned, this can also happen when a total IGA test isn't done.

I found at Disney springs and Disney they have specialist that when told about dietary restrictions they come and talk to you ,explain cross contamination measures tsken and work with you on choices. Its the one place I dont worry once I've explained I have celiac disease.  Thier gluten free options are awesome.

Have you tried Pure Encapsulations supplements? This is a brand my doctor recommends for me. I have no issues with this brand.
