Capsule Endoscopy?

ok, so my gastro wants to follow up my diagnosis (which happened in August) with a capsule endoscopy. in his words, it's to establish a baseline of images of what my small intestine looks like in order to be able to track any changes that might occur in the future. (i.e. it is not for the purpose of diagnosis -- been there, done that.)

the problem comes in in that my insurance company will not pay for it. it will cost $900. i do not have $900. do y'all think this is worth it? i want to do all i can to be healthy and take all the right steps here, but i have to be sure about this before i sign away this much money. i'm just a broke grad student, so this is a big decision. any thoughts would be greatly appreciated!!

Happy Halloween! :D

If I were in your situation, I'd skip it. There are other ways of monitoring your health - if you're feeling healthy, you probably are. I was dx'd old school - diet challenge. None of my drs (the original one who dx'd me) or subsequent dr's have ever mentioned looking into things, as it were. I think it's easier for the dr's to look at images, but it's not his $900 bucks paying for it. Your health can be monitored by what your body is telling you - if you're feeling better on the gluten-free diet, then you're getting healthier. Energy levels, appetite, skin, regularity, all can point to health. He could order blood work to see if your hormones, iron, and nutrition is being absorbed properly. That would give him a baseline and be much much cheaper.

When Forum George sees this he will probably reply. There is a test that he was talking about the other day but name of it escapes me now (it's late) I will look it up. I know it is used for monitoring 'leaky guy' which is what the capsule endoscopy will do. That is aa worthy tool for diagnosis - not a monitoring tool. Why - because it is overkill. You can find out the same thing in a much less espensive way. Claire

Had another thought re: test. (sorry, couldn't keep focused last night - all these little creepy things were at my door for candy) - He wants the test as a baseline, right? Well, that means if he doesn't order the test again in the future, then what's the point of getting the test now? Meaning, this would be the FIRST $900 you'd have to plunk down out of your own pocket. Just a thought.

I have not had endoscopy yet, and if I knew insurance wouldn't pay for it, I wouldn't do it unless I was having some pretty severe continuing symptoms and had already tried an elimination diet, allergy and stool testing and other blood tests. I am still symptomatic, but I am closing in on the cause without endoscopy with an elimination diet. If I cut out everything and still have symptoms regardless of what I'm eating, I may have it done just to rule out something super bad. But if I had something super bad, it would probably be showing up in my blood work and show up in stool tests.

No. Skip it. I am firmly against any uneeded test. ALL test have side effects and risks, no matter how "rare", why risk it when you get no new info. Tell him to swallow the pill himself :)

I really don't know if the test is worth it or not, so I will comment on insurance. I have some children with special needs. I had problems with insurance paying for some of their services. I protested their decision by writing a letter along with my doctor's recommendation. I was granted the services. A lot of times insurance says no, and all you need to do is protest it, and you can get them to pay for things.

I agree with previous post. I wanted a blood test that my Dad's cariologist said I needed. Went to dr. to get it, they informed me ins. wouldn't cover. Called insurance company and got it approved, as long as test was done in dr. office. No problem I think so I go back to dr. for the test. Oops! Test must be done in lab due to freezing requirements for sample. So I call insurance company again and they finally approved it to be done in the lab. Problem was it for folic acid deficiency and by the time I got the test, I was too low on folic acid - as well as other things due to malabsorption issues. I started trying the get the test in Jan. but it took until August to get the approval. Until I get the malabsorption thing under control we won't know if my folic acid def. is from celiac disease or something else! What a pain insurance companies are!! But I'd fight tooth and nail for the insurance company to pay for the capsule test if I were you. They routinely deny coverage when they shouldn't so I like to make them pay every penny possible.

I don't know if I am getting sufficient Omega Threes. I read about  phosphotidyl choline may cause heart issues. I will have o do further research on heathy Omega 3 supplements or from foods. Is there a blood test that can tell you everything level in your system such as Thiamine, Benfotiamine levels etc? Thanks

If lectins were my problem, I would react to wheat germ (the highest source of wheat lectins) and beans. I don't. I only react to bread and pasta, which are the highest sources of gluten. Therefore, my issue is wheat-specific (Gluten/ATIs), not a general lectin issue.   I have eaten a supposedly high lectin diet (I say supposedly because lectin content in these foods is greatly reduced by proper cooking and I eat very few of those foods raw, and even then, rarely!!) for years. My health has improved greatly on my whole foods plant forward diet. I have asked all my drs and a registered dietician about my diet, asked if eating such a high amnt of fiber might interfere with the digestion of any other nutrients and the answer has always been NO.     while doing the gluten challenge I did not eat ANY wheat germ (since it doesn't have hardly any gluten, and I was too sick from the bread and pasta to want to eat much anyway) I will NOT put that poison in my body again. That was a horrific experience and if this is what most celiac patients have to deal with, I am very sorry for them I don't care if I have celiac or NCGS I won't intentionally cause myself that much pain and suffering it's not worth it.

@catnapt,  Wheat germ contains high amounts of lectins which are really hard to digest and can be irritating to the digestive tract.  They can stimulate IgG antibody production as your blood test shows.   Even beans have lectins.  You've simply eaten too many lectins and irritated your digestive tract.   You may want to allow your digestive tract to rest for a week, then start on gluten in "normal" food, not in concentrated vital wheat gluten. This explains it well: Lectins, agglutinins, and their roles in autoimmune reactivities https://pubmed.ncbi.nlm.nih.gov/25599185/

I take Now B-1 (100 mg) Thiamine Hydrochloride, and Amazing Formulas L-Tryptophan (1000 mg).   Both are gluten free and free of other allergens.  I've taken them for a long time and haven't had a problem with them. I take Vitamin A from BioTech called "A-25".  It's gluten and allergen free and made in the USA.  It's a powder form of Vitamin A.  I was having trouble digesting fats at one point, but found I tolerated the powder form much better and have stuck with it since.   Tryptophan and Vitamin A help heal the intestines as well as improves skin health.  I get Dermatitis Herpetiformis and eczema flairs when my stomach is upset.  So I'm healing the outside as well as the inside.   I take one 1000 mg Tryptophan before bedtime.   With the Thiamine HCl, take 100 mg to start.  If you don't notice anything, three hours later take another. You can keep increasing your dose in this manner until you do notice improvement.  Remember not to take it in the evening so it won't keep you too energized to sleep. When I first started Thiamine HCl, taking 500 mg to 1000 mg to start was recommended.  If you've been thiamine insufficient for a while, you do notice a big difference.  It's like the start of a NASCAR race: Zoom, Zoom, turn it up!   This scared or made some people uncomfortable, but it's just your body beginning to function properly, like putting new spark plugs in your engine.  I took 1000 mg all at once without food.  It kicked in beautifully, but I got a tummy ache, so take with food.  I added in Thiamine TTFD and Benfotiamine weeks later and felt like I was Formula One racing.  So cool.  You may feel worse for a couple days as your body adjusts to having sufficient thiamine.  Feels sort of like you haven't cranked your engine for a while and it backfires and sputters, but it will settle down and start purring soon enough.  Adjust your dose to what feels right for you, increasing your dose as long as you feel improvement.  You can reach a plateau, so stay there for several days, then try bumping it up again.  If no more improvements happen, you can stay at the plateau amount and experiment with increasing your Thiamine TTFD.  It's like being your own lab rat.  LoL Yes, take one Benfotiamine at breakfast and one at lunch.  Take the B Complex at breakfast. Take the TTFD at breakfast and lunch as well.  I like to take the vitamins at the beginning of meals and the NeuroMag at the end of meals.   You may want to add in some zinc.  I take Thorne Zinc 30 mg at breakfast at the beginning of the meal.   Are you getting sufficient Omega Threes?  Our brains are made up mostly of fat.  Flaxseed oil supplements, sunflower seed oil supplements (or eat the seeds themselves) can improve that.  Cooking with extra virgin olive oil, avocado oil, or coconut oil is also helpful.   @Wheatwacked likes phosphotidyl choline supplements for his Omega Threes.  He's also had dramatic health improvement by supplementing thiamine.  You're doing great!  Thank you for sharing your journey with us.  This path will smooth out.  Keep going!
