If you haven't had the full bloodwork done, I would suggest that you do that right away. I would follow that by starting the gluten-free diet once the blood draw is done, unless you plan to follow with another endoscopy. (Did your previous endoscopies include biopsies? Based on family history, and symptoms, yes, you should have him tested. No, the results aren't quite as reliable in children that age, but it only means that if there's a negative, you check him again periodically (if there's continuing symptoms, I'd say yearly, but I don't know how well a doctor would buy that). I'd say just go cheeseless. Follow Your Heart is one of three soy based cheeses that don't have casein (that I've been able to find), and I hear it melts well, but you can make cheeseless pizza by using a good tasting sauce (I make my own), and good, flavorful ingredients. (Though every pepperoni I've seen has dry milk in it.) -

I would encourage you to talk to your doctor (I presume you're dealing with one in Germany) and find a local support group. With the number of people over there who seemed to speak English when I visited, chances are, at least one person in the support group will speak English, and can help translate for you. -
If you think it's DH, call around to dermatologists and ask them if they have deal with any celiac patients that they've done skin biopsies on. No reason not to call around first; you're shopping for a service.

My results on the blood tests were very inconclusive (though I had been gluten-free for two weeks), but my results on the diet have been good, and gluten challenges have produced obvious symptoms. -

Kraft will label any ambiguous ingredient if it has gluten. So, if the modified food starch was made from wheat, the ingredient would say modified wheat starch. If an item could be ambiguous, and does not specify that it came from a gluten containing grain, it's gluten free. -

They're not specialty foods, for the most part, but still gluten-free and tasty! 1) Food For Life sprouted corn tortillas (better than regular store bought, though nothing's as good as homemade) 2) Thai Kitchen Chili Paste 3) Lundberg Farms Rice Cakes 4) Trader Joe's Dried Mango 5) meat! :-) -
Entirely depends on the cookware. Stainless steel should be fine being thoroughly washed. Wooden spoons not so much, and collanders definitely not. And non-stick pans should never go in the dishwasher anyway, so I'll assume those aren't even being asked about. Anything that's not insanely easy to thoroughly clean without having a scrap of anything left...
