No Follow Up Planned, Should I Worry?

Because my dr "strongly believes I have celiac though it didn't show up yet" -it isn't on any medical charts, so I have no follow up scheduled. Should I be concerned of vitamin levels? I'm only aware of low ferritin because I was seeking answers for hair loss prior to a celiac dx. Any concern or blow it off like my dr has? It's been a long battle here, where celiac is almost unheard of. I've been gluten-free for about 9 months now.

I don't use doctors much, but I surely wanted some help when I was diagnosed.  I would at least keep an eye on that ferritin level while taking natural supplements.  My ferritin level seemed really tied to foggy brain and fatigue.  If you feel really good, perhaps it would be fine for you to consider not to.  Otherwise you may want to consider if you have other food intolerances, thyroid problems, or osteoporosis.

I had negative blood work. Based on your biopsy, genes and if you're feeling better gluten-free that should confirm a celiac diagnosis. Open Original Shared Link I don't see why you're "officially and unofficial celiac." Yes, you should have your vitamin levels checked and see what you may need to supplement. If your doctor won't check, and won't consider you celiac it may be time to fire him and find a new one. Remember, doctors work for you because you're paying them. See one you like who takes you seriously and treats you well.

I guess he officially diagnosed me, but nothing 100% confirmed it. I'm seen as the drama queen in his office, so I'd rather not go back there and get vitamin levels checked. But I find myself in a strange position to ask for a vitamin level check because my dr said I'm celiac, though I have no definite dx. I'm thinking I'll talk to my GYN in July during my annual and see if she'll order them.

OB/GYNs like that stuff! Tell them you don't really have a regular doctor. Get your thyroid checked while you are at it. Mine will do cholesterol, blood sugar, etc if they think they are the only doctor you are seeing.

Thanks! I sure hope she does!

I hope she'll run that blood work for you!

Celiac disease is the most likely cause, but here are articles about the other possible causes:

Hi @Atl222 As @trents points out, there could be many reasons for this biopsy result.  I am interested to know, is your gastroenterologist concerned?  Also, are your blood tests showing steady improvement over the years? I remember when I had my last biopsy, several years after diagnosis, mine came back with with raised lymphocytes but no villous damage, too! In my own case, my consultant wasn't remotely concerned - in fact, he said I might still get this result even if all I ever did was eat nothing but rice and water.   My coeliac blood tests were still steadily improving, albeit slowly, which was reassuring.

Welcome to the celiac.com community, @Atl222! Yes, your increased lymphocytes could be in response to oats or it could possibly be cross contamination from gluten that is getting into your diet from some unexpected source but not enough to damage the villi. And I'm certain that increased lymphocytes can be caused by other things besides celiac disease or gluten/oats exposure. See attachment. But you might try eliminating oats to start with and possibly dairy for a few months and then seek another endoscopy/biopsy to see if there was a reduction in lymphocyte counts.

This is a solid, well-reasoned approach. You’re right that “koji” by itself doesn’t indicate gluten status, and the risk really does come down to which grain is used to culture it. The fact that you directly contacted Eden Foods and received a clear statement that their koji is made from rice only, with no wheat or barley, is meaningful due diligence—especially since Eden has a long-standing reputation for transparency. While the lack of gluten labeling can understandably give pause, manufacturer confirmation like this is often what people rely on for traditionally fermented products. As always, trusting your body after trying it is reasonable, but based on the information you gathered, your conclusion makes sense.
