Does this look like DH?

Hello-

I have read several posts asking this, and searched through google images, but I was hoping for live-person input.

I originally was hospitalized with what they thought was cellulitis. Sent home with antibiotics twice (it kept popping up in different locations) before being admitted twice. Skin biopsy during hospital stay showed either arthropod bite or drug reaction (house has been . That was the beginning of March.

I have since seen an allergist, dermatologist & have an upcoming rheumatology appt (probably a waste of time - ANA & antidsDNA negative). I have been diagnosed with rosacea, I have stopped my birth control (in case it was drug reaction - a month later still no improvement). My dermatologist has me wearing deet everyday, which I’m about to discontinue since it’s also not working & im sick of the smell.

Anyways, the original biopsy was from the lesion, I’m thinking of having derm do DH biopsy when I follow up & asking rheumatology when I see them next week to send me for a celiac panel. Just want to know if you guys think it’s worth my time.

hopefully that one works

In those photos I don't see this presenting bilaterally. I only see one shoulder but not the other, 1 hand but not the other, 1 elbow but not the other, etc....

You haven't told us how it acts, how it feels. We can not make any kind of determination from photos alone.

Looks more like bug bites that have been scratched causing some surrounding redness and irritation, but I am not a doctor.  It does not seem like hives either.   The rosacea?  I have it.  Mine is definitely triggered, not by gluten, but other food intolerances like onions or garlic which have never resolved despite healing fromceliac disease.

It’s popped up on both elbows, both shoulders, both feet & has now popped up on one hip (to be honest, I got bad at taking photos since there’s so many places). The spots are hot to the touch at first, they itch & sting then after about a day they start feeling more bruise like. They look almost like a wart at first then start to scab over (with & without scratching) & leave scars (dark red)

The rosacea is kind of what lead me this direction. Ive been a google fiend since dealing with this, & with celiac having so many possible symptoms I of course found some of my symptoms in the list.

My drs are mostly at the stage where they’re trying to figure out what it’s not. That’s where the deet comes in that I’ve een wearing for a week with new bumps still coming up daily.

The rash and bilaterally looks so muc like my sister’s. She just got her biopsy back positive for dh. She also had positive antibody test and genetic markers. Dermatologist argued with her about needing the biopsy, oh yes she was negative on endoscopy. I also have celiac positive endoscopy and antibodies and of course the genetic markers.

I think you ought to get the dh biopsy done properly this time & get the serum panel. Those of us with dh test negative on the serum panel 60% of the time so do be sure to get the dh biopsy and keep eating gluten every single day until then otherwise you will most likely get a false negative.

@lizzie42, So, I'm confused with what you are uncertain about. Do you really have any doubt that your 5 year-old son has celiac disease? Is it because he isn't exhibiting anemia and the rash as did your daughter? The genes are certainly there and the antibody testing certainly indicates celiac. And given the fact that he was on a reduced gluten diet and on a steroidal medication (which would likely suppress immune responses) do you really have any doubt? If you can't afford the GI consult with endoscopy/biopsy, why would you consider a gluten challenge after removing the remainder of the gluten from his diet? Have you considered that his asthma and "sickness" may be tied to celiac disease?

These are my sons results. My daughter was high positive on everything and maxed out the tests in addition to being anemic, bad rash, etc.  Yes, he has been eating mostly gluten free (not completely). He was also on a high dose of budosinide at the time of testing due to some asthma and sickness. I didn't realize that affects the results.  This test along with my daughter being so sick with it prompted the pediatrician to diagnose him. We can always do a gluten challenge in the future. We will do that with our 1 year old. We will have her eat school lunch when she goes to prek and then test after a couple months.

@lizzie42, Are the results you posted above for your daughter or for your son? If these are for your 5 year-old son, they are strongly positive and definitely suggest celiac disease. The ttg-iga is the centerpiece of celiac disease antibody testing and at 59 it is a strong positive. If you are wondering why some of the other tests are negative, you need to know that is normal. Seldom are all the antibody tests positive on a complete celiac panel.

If your son is only 5 and his sister is diagnosed celiac disease, is it a possibility that he has been eating a majority gluten free diet with his sister.  If this is the case his result would be low or negative.  The choice would be yours as to whether to put him in onto a full gluten diet for 4 -6 weeks, handle and NOTE the changes in his diet, behavour and body symptons.  Then to test again and see if there is a difference.  The other choice is to just continue on the gluten free diet without any medical diagnosis.  Remembering that should you decide to actually have the test in the future you will have to run the gluten challenge.  Please let us know how we can support you

@cristiana Yes - just to add to this: my ferritin went down after my first 2 infusions (which were 8 months ago), so I got more infusions a couple of weeks ago. From those alone, I'm feeling better. It's a work in progress! Be patient! I was also taking iron supplements the entire time.
