Enterolab Severe Fecal Fat Malabsorption- pancreas issues or celiac?

Lisa M M

Hello All,

This is my first post and I am curious if anyone has pancreas issues and saw a very severe fat malabsorption. Specifically - im worried about the malabsorbtion being related to pancreatic cancer.

i did 23 and me and I have celiac genes, and an IgG test came back positive for wheat and oats.  When i did this enterolab, i had reduced my gluten for awhile, but it was not at zero.

Enterolab writes "Pancreatic insufficiency as the primary cause of fat malabsorption usually causes significant elevations of fecal fat values, usually into the moderate (600-1000 Units) or severe (>1000 Units) ranges."

1) Gluten Sensitivity Panel

Anti-gliadin IgA      93 Units   (Normal Range is less than 10 Units)

Anti-tissue Transglutaminase IgA      20 Units   (Normal Range is less than 10 Units)

Quantitative Microscopic Fecal Fat Score      2838 Units   (Normal Range is less than 300 Units)

I would follow up with a endoscope and biopsy to confirm celiac, please keep eating gluten in the mean time. As for you pancreas issues, I also have a bit of a enzyme issue, never checked for fecal fat....I just take 2x the amount of supplemental pancreatic enzymes to help me break down my food. I got a generic OTC one that uses porcine enzymes...I have some post and articles about enzymes issues I will see about digging up and linking for you.

Celiac experts are not supportive of Enterolab for celiac testing.  Consider getting standard celiac testing.   Learn more at

Pancreatic is sufficiently is commonly linked to celiac disease.  If you are diagnosed with celiac disease, doctors are more likely to keep an eye out for cancer down the road.  It sounds like you had a family member who had pancreatic cancer.  Supplemental enzymes can help, but getting to the root cause is even better.  I will not repeat the rest of Ennis’ advice.

You are worried that you might have cancer?  Get to a doctor!  Get legitimate medical testing and care!  I wouldn’t try to self treat cancer if I thought I might have that.

I agree with @trents, could be worth having another scope. Oddly enough I'm in the same boat except my current odd sensation is near my appendix, I think I'll request a colonoscopy for peace of mind.

Yes, take this up with your GI doc during the next visit. It might be a good idea to get an upper GI scoping done if you haven't had one done since diagnosis.

I will be doing in the new year. I had a thought that it might be Diverticulitis but I have no other symptoms related to that.

Have you mentioned it to your gastroenterologist?
