a LOT of things can cause general fatigue. some of the ones I believe have affected me: testosterone deficiency, possible vitamin deficiency, sub-chronic viral infection, sub-chronic asthma flare, seasonal affective disorder, dysthymia, insufficient exercise, and stress. it's well worth talking to your doctor, but it can be tricky to get a doctor to take... -
I'd definitely try calling back. If nothing else ask the "ok, you can't see me for six months. what do I do about the life-altering and work-threatening symptoms/pain in the meantime?". That often, at least, gets them to listen to you and understand that stupid position they are asking you to take.

you might also consider getting a deep tissue foot massage. it sounds like you've got excess nerve firing going on in your toes/feet, and a deep tissue massage may be able to help relax the muscles that are first tightening and staying tight, if you catch it early. this is, at best, an educated guess, though.

It's important to note that European wheat starch is different than American wheat starch, and is more highly refined, and - when approved for use in gluten free foods - contains less gluten than American wheat starch.

If you used your mill to grind wheat flour, it may be hopelessly contaminated. :-( Is the restriction on vinegar due to grains? If so, would apple cider vinegar, rice vinegar, wine vinegar, or balsamic vinegar work? It's not the full panel, but it's not bad. That second test is the total serum IgA. You need to know that in order to be able to properly read the anti-gliandin IgA and tissue transglutimase IgA. You may want to focus more on beans/lentils and veggies, though eggs are a good, cheap source of protein. You might also want to look into the gluten free grains, which can be obtained more cheaply than specialty prepared products. Millet, in particular is very good. But asthma isn't necessarily CAUSED by gluten intolerance, so it's not reasonable to expect going gluten free to make it go away. Stomach problems (particularly stool problems, bloating, gas, and abdominal pain) CAN BE caused by gluten, so, if that't the only thing going wrong, it IS reasonalbe to expect that going gluten free will make it go away. Tylenol won't do anything - it doesn't address the pain receptors in the area. Ibuprofen or naproxyn sodium will, but if it bothers your stomach even if taken with food, that's not going to work either. Some other things that work are heat, as previously mentioned, massage, or sex. Obviously the last two are hard to implement at work :-), but if you can...

Gluten free means gluten FREE! No contamination, or it's just like you're not following the diet. It's a chemical process that's damaging the intestines, and ingesting any gluten will set off that chemical cascade. I've been gluten-free for two and a half years, and it does get better. I never had the severe symptoms, so I got better fairly quickly, but don't forget that there are other medical issues out there that can make you feel under the weather.
