Biospy Report Help With Understanding It

Hi I had a endoscopy on 12/21 and I got the results on Friday. I have had one positive and one negative biospy in the past year and neg bloodwork. These results I think are another positive. I was just looking for someone elses input as I think I have celiac and I really wish my doc would see it too, but since my bloodwork is negative he doesnt want to see it.

Part 1:Duodenal Biospies:

Fragments of duodenal mucosa with areas of villious blunting, intraepithelial lymphocytes, along with acute and chronic duodenitis.

Note: Although nonspecific, these findings may be associated with celiac diease.

Part 2: Gastric biospies:

Fragments of gastric fundic and antral muscosa with chronic inflammation along with areas of regenerative/reactive glandular atypia.

Note: No organisms suggestive of H. pylori seen on Alcian yellow stain.

Part 3: Esophageal biospies:

Fragments of esophageal squamous mucosa with basal layer hyperplasia and slight subacute and moderate chronic inflammation.

Note: No obvious intestinal metaplasia seen on H E or Alcian blue stains. There are between 5 and 10 eosinophils per selected high power field. Clinical and endoscopic correlation recommended for evalutaion of eosinophilic esophagitis.

Since increased intraepithelial lymphocytes are associated with early stage celiac (marsh 1, for example)

AND villous blunting is certainly a celiac symptom

AND you have chronic inflammation

AND you already had a POSITIVE biopsy (geesh, how many biopsies are they going to put you through?? :blink: )

it sure would make me think I have Celiac.

MANY people have NEGATIVE blood work and positive biopsies. What's your doc's problem here????? :rolleyes:

Usually, in the AP report, the most important part i that of the duodenal biopsy --- and usually the doctors / pathologists describe those duodenal biopsies with more exactly numbers, to better understand if you have or not an enteropathy: "areas of villious blunting" - usually they write if the ratio vili/cripts is 3:1, 2:1, or less than this, or more than this... "intraepithelial lymphocytes" - usually the normal rate is 20-25 per 100 epithelial cells, depending on your age, its better to describe the number of intraepithelial lynphocytes that were finded... Did you get?

If you read some of those numbers on your report, fell free to tell me!!! With that information, it is better to grade the stage of your disease. But im sure that you may have some pathological signs (remember - they all are not specific) of celiac disease - the duodenitis, the vili blunting, and, depending on the number of intraepithelial lynphocytes, also inflamatory intracellular reaction - changes.

Hi all!

Thanks for the responses but the doctor decied since this is the second positve in a year that it 'must' celiac! So he said to try a gluten free diet! woooo whoo!!! thanks it has been a long 2 years!

      This is why Daura Damm can be a sponsor here--at 3ppm or less it is gluten-free, and it's doubtful that anyone with celiac disease would ever have issues with such levels. Some people may be reacting to the yeast in the beer, but I seriously doubt that such beers could trigger elevated antibodies or villi damage--the science says such levels won't trigger...

      Hello, I was newly diagnosed in January of this year (2025).  Since then I have been strict about staying gluten free and only cooking at home.  I started feeling better in July while gradually resuming close to my normal routine of activities and athletics. September and October were extremely stressful due to a new home build being finished and moving.  My spouse and I take care of his mom who has advanced dementia and have been since 2021.  We did all the moving as well as get the other house on the market for the month of October.  Since earlier this month I feel like I did back in the early stages of my diagnosis.  Almost all of my symptoms have come back except for the bad abdominal issues.  I haven’t changed my diet or supplements since January and wonder if the stress has caused a set back? Any thoughts are helpful.  Thanks
